墩柱首件施工方案1、编制依据及目的1.1 编制依据(1)宜宾至叙永高速公路两阶段施工图设计；(2)公路工程质量检验评定标准 JTG F80-1-2004； (3)公路桥涵施工技术规范 JTG/T F50-2011；(4)混凝土结构工程施工质量验收规范 GB 50204-2002；(5)钢筋焊接及验收规程 JGJ-18-2012；(6)公路工程技术标准 JTG B01-2003； (7)公路工程施工安全技术规程 JTJ 0763、-95； (8)公路桥涵设计通用规范 JTG D60-2004。 1.2 编制目的编制此方案目的有以下几点：一是指导、策划、组织双桥互通D匝道大桥3#墩墩柱首件施工；二是摸索墩柱施工中的各项工艺流程，技术要点；三是合理组织调配各类资源，实现资源优化配置；四是推广施工中的优点，总结改进施工中的缺点，为后续墩柱工程提供良好的借鉴和指导。2、工程概况宜叙高速20分部位于泸州市叙永县境内，路线起点桩号为：K91+000，终点桩号为：K94+534.372，主线路线全长3.534km。宜叙高速全线采用高速公路标准建设，设计速度80km/h，路基宽度24.5米。20分部起点位于叙永县龙凤乡凤林村，设永宁河4、大桥（K91+044-K91+477）横跨永宁河，设叙永龙凤互通与叙永国道321相连、双桥枢纽互通与纳黔高速相接南凌互通相连接为叙永境内控制性工程，路线跨越叙永县龙凤乡凤林、凤池、双桥三个村落。双桥枢纽互通DK0+289.726 D匝道大桥位于双桥互通A匝道上，为顺河沟接A匝道大桥而设。本桥起止桩号：DK0+168.726-DK0+407.226，中心桩号：DK0+289.726，全桥长为238.5m。该桥荷载的等级：公路-I级；桥面净宽：7.5m。全桥分为3联：（3*25+4*20+4*20）m；上部结构第1联采用预应力砼预制小箱梁，桥面连续，第2、3联采用预应力砼现浇箱梁：下部结构起点桥台5、采用桩柱式桥台，桩基础，止点接A匝道桥；桥墩采用柱式墩，灌注桩基础。墩柱首件工程选择双桥枢纽互通DK0+289.726 组织机构为保证首件工程顺利、安全、优质、高效完成，按照“统一指挥、统一协调、职责明确、提高效率”的原则，项目部成立“墩柱首件工程”施工组织领导小组，全面负责墩柱首件工程的6、动员、组织、控制及总结等工作。组长：赵明攀副组长：魏铭 成员：缪雨、但照银、张怀、冷君臣、毛勇、汤叶凯、徐春森 3.2 组织机构职责另根据工程中的控制程序和质量要求，围绕技术准备、质量保障、物资保障、现场管理等成立7个职能小组，各司其职分头按照本方案内容和计划开展工作。职责和分工：(1)动员策划组（负责人：赵明攀）负责首件工程的组织策划和动员工作，负责后勤保障、接待和服务等工作，确保首件工程顺利实施。组员：卿光娅(2)技术保障组（负责人：魏铭）负责墩柱首件工程的技术指导，负责墩柱施工技术交底工作，负责首件工程的资料汇总、上报，组织首件工程总结等工作。组员：张怀、冷君臣(3)物资保障组（负责人：7、杨成武）负责钢筋、砂石骨料、模板、水泥、粉煤灰、外加剂等材料的组织与供应。组员：宋克军(4)现场组织组(负责人：刘应高)负责首件工程的现场实施，负责人员、设备的组织和安排，负责首件工程各工序的施工安排，负责首件工程混凝土的供应。组员：肖汉一 首件工程施工准备4.1 施工便道该工程部位混凝土运输使用凤池村混凝土道路。4.2 施工用水拌合站施工用水使用拌合站地下水。4.3 施工用电拌合站施工用电使用120KW发电机发电。4.4 材料准备4.4.1 钢筋本工程所用钢筋在双桥互通钢筋加工场中进行加工，钢筋下料前要清除钢筋表面的油渍、漆污、水泥浆和利用锤敲击能剥落的浮皮、铁锈等。在除锈过程中或使用前，若发现钢筋表面的氧化铁皮鳞落现象或麻坑现象严重，并且已伤蚀钢筋截面时，不得将此钢筋用于施工中。所有钢筋在使用前要调直，所用直径20、25的三级钢必须用调直机调直，9、所用直径10的一级钢使用卷扬机冷拉调直或调直机调直。钢筋笼按图纸钢筋构造图中加工完毕后由钢筋运输车运输至施工部位人工组编。4.4.2 模板钢模板进场后必须进行准入报验手续，接着进行模板试拼装，主要检查模板拼缝质量、连接螺栓的安装难易情况和模板各连接部分是否符合设计结构尺寸要求，模板验收合格后方可投入使用。模板组拼完成并检验合格后将所有钢模分套按顺序标号。4.4.3 原材料原材料的选用要严格要求，水泥采用宜宾瑞兴实业有限公司P.O42.5R水泥；砂采用兴文大弯头料场机制砂；碎石采用兴文大弯头料场碎石粒径为（4.75-19mm、19-31.5mm）；粉煤灰采用贵州省习水县火电厂级粉煤灰；外加剂采用10、北京方兴建材有限公司JF-B聚羧酸高效减水剂；进场时对碎石压碎指标、颗粒分离、含泥量、级配、表观密度进行检验。特别要严格控制砂的含泥量小于3，石子要求无黄皮水锈。4.5 技术准备技术人员认真学习设计图纸及公路桥涵施工技术规范，并做好技术交底。并跟踪现场施工情况做好施工信息的反馈。对于图纸中与施工实际情况不符的，及时通过图纸反馈表的形式逐级上报。4.6 墩柱施工工艺5.1 施工工艺流程施工放样钢筋制作与安装模板安装、加固复测三心混凝土浇筑拆模覆盖养护5.2 测量放样在吊装钢筋笼之前必须对墩柱与地系梁的连接部进行凿毛处理，并用清水冲洗干净。然后利用全站仪现场定出墩柱纵横轴线、四角坐标及高程，并对系梁位置进行放样，系梁标高测量。在地系梁上用红漆作上明13、显的标记，再将墩柱边线弹出墨线，以便钢筋笼吊装时能够准确、快速地对位。5.3 钢筋笼制作与安装墩柱首件工程钢筋笼在双桥互通钢筋场统一加工制作，制作完成并自检合格后报现场监理验收，集中存放在钢筋场，存放区采用下垫上盖，保证钢筋笼不变形。(1)施工中应严格按照施工图及施工规范要求绑扎钢筋；钢筋接头采用搭接电弧焊，两钢筋搭接端部应预先弯向一侧，使两接合钢筋轴线一致；接头双面焊缝的长度不小于5d，单面焊缝的长度不小于10d；接头处焊缝应饱满，不得有裂纹，不得有深度大于0.5mm的咬边；接头处的轴线偏移不得超过钢筋直径的0.1倍，同时不得大于2mm。(2)在主筋上绑扎箍筋时，先在主筋上用粉笔划出箍筋位置14、线，并严格按设计要求进行箍筋的绑扎固定。(3)立柱钢筋绑扎至系梁位置后注意进行系梁的钢筋焊接安装。钢筋安装好后应对其规格、数量、排距、长度、足位置进行检查，确保满设计要求。(4)钢筋绑扎时扎丝头一定要向内弯，防止扎丝伸入保护层浅部。(5)钢筋笼安装前应清除粘附的泥土和杂物，保证钢筋与混凝土紧密黏结。5.4 模板安装模板是墩柱外观质量的保证条件，要求模板刚度要符合要求，表面光滑平整，拼缝严密不错台。(1)钢模由模板加工厂严格按照设计图纸尺寸要求制作，模板加工好后由技术部、设备物资部、质量管理部进行自检验收合格后报监理检查合格方可使用。(2)模板在使用前后都存放在平整的场地内，临时存放时将模板垫平15、，以避免模板变形。(3)施工前模板内均匀地涂抹一层CXP脱模剂。(4)按照图纸对墩柱模板进行拼装，拼装时模板的接缝间设一层双面胶,要求模板间的连接应紧密、牢固。(5)墩柱钢模板吊装采用汽车起重机吊装，起吊时模板应保持竖直。将模板吊至指定位置后，模板工及时对模板进行安装固定。(6)模板安装完成后用全站仪检查上口偏位，通过缆绳调整使上口位置正确。用垂球检查模板垂直度，如不符合要求应及时调整。(7)为防止浇注混凝土时出现漏浆，钢模板定位前应在垫层上根据墩柱外形尺寸在模板外围制作找平砂浆带，钢模安装完成后在模板底部用水泥砂浆进行封堵。5.5复测“三心”利用十字交叉法确定墩柱中心、模版中心、钢筋笼中心，16、使用铅锤检测墩柱中心、模版中心、钢筋笼中心是否在一条垂线上，若不在则使用全站仪纠偏，检查偏离原因并调整。直至三点一线，再进行下一道工序。5.6混凝土浇筑混凝土由罐车运至墩柱施工地点，用汽车起重机吊料斗方式将混凝土吊至墩柱顶端，使用串筒下料。 (1)混凝土浇筑要分层浇筑，分层厚度30cm左右，振动棒按有效直径60cm分布，每浇筑一层，提升一层，振捣时采用四周向中心集中的方式，以减少混凝土表面的气泡。(2)为满足每层浇筑厚度控制在30cm，汽车起重机起吊混凝土料斗制作为0.8m，料斗内0.5m位置处划线，每次每斗起吊混凝土0.5m。(3)混凝土施工前先在墩柱内进行洒水湿润，但不得使模板内有积水。然17、后再铺上12 cm厚的1:2水泥砂浆，以保证新旧（与承台）混凝土间能有效地连接。(4)混凝土施工前、施工过程中都应定时进行现场坍落度检测，坍落度应严格控制在140180mm。(5)振动器振捣时应符合下列规定：与侧模板保持510cm的距离；振捣棒每40cm做一标记，振捣完成时抽出40cm，以保证振捣棒保持插入下层混凝土510cm；操作上要求“快插慢拔”；应避免振动棒碰撞模板、钢筋及其他预埋件。(6)为防止混凝土产生离析，混凝土倾落高度不得大于2m，本工程拟定混凝土浇筑过程串筒口离浇筑面最高为1.3m。 (7)对每一振动部位，必须振动到该部位混凝土密实为止。密实的标志是混凝土停止下沉，不再冒出气泡18、，表面呈现平坦、泛浆为止。 (8)混凝土的浇筑连续进行，如因故必须间断时，其间断时间应小于混凝土初凝时间。浇注过程中，工人应及时检查保护层垫块是否脱落或布设足够，不足时应及时增设。(9)浇筑混凝土期间，设专人检查模板的垂直度及支架的稳定性。当发现有松动、变形、移位时，应及时复测，调正。5.7 养护混凝土脱模后，应及时使用塑料薄膜进行缠绕和覆盖，覆盖前应洒水养护。养护期间必须保证薄膜内有大量的凝结水凝结在薄膜上。洒水养护时间不得少于7天。养护结束后及时拆除缠绕薄膜。6.施工进度计划墩柱首件计划于2014年4月4日开工，计划3日浇筑完成。其中钢筋笼安装半天，模版安装一天半，混凝土浇筑一天。7.施工19、质量保证7.1施工质量目标严格执行招标文件、设计图的技术要求，执行交通部公路工程质量检验评定标准等有关技术标准；工程合格率100%，单位工程一次性验收合格率100%，分项（工序）工程合格率100%。7.2质量保证措施7.2.1混凝土工程(1)混凝土搅拌采用强制性搅拌机、电子计量系统、含水率实时监测系统，耐久性混凝土搅拌符合相关标准规定。对拌合物的坍落度、扩展度、泌水率、含气量等进行测定，保证良好的和易性。(2)混凝土运输运输道路平顺畅通，选用两辆10m专用混凝土运输车。每辆车装载8m混凝土，两辆车连续浇筑，第一车送出混凝土10斗（5m）时第二车开始打料。(3)混凝土浇筑本工程混凝土浇筑采用汽车20、起重机输送，由运输车将混凝土运送至施工区料斗，再由汽车起重机起吊输送进仓内。浇筑混凝土前，仔细检查钢筋保护层垫块的位置、数量及其紧固程度，并指定专人作重复性检查，以提高钢筋保护层厚度尺寸的质量保证率。绑扎垫块和钢筋的铁丝头不得伸入保护层内。保护层垫块的尺寸须保证钢筋混凝土保护层厚度的准确性，其抗腐蚀能力和抗压强度不低于构件本体混凝土。混凝土的浇筑采用分层连续推进的方式进行，当因故间歇时，其间歇时间不大于前层混凝土的初凝时间或能重塑的时间，最大浇筑间隙时间不超过45min，不得随意留置施工缝。浇筑新混凝土前，对水平施工缝宜在旧混凝土面上铺一层厚1020mm、水胶比比混凝土略小的1:2水泥砂浆，或21、铺一层厚约30cm的混凝土，其粗骨料宜比新浇筑混凝土减少10%。(4)混凝土振捣混凝土振捣时不得碰撞模板，保护层部位振捣时要距离模板510cm，不得贴着钢筋振捣。混凝土振捣按事先规定的工艺和方法进行，混凝土浇筑过程中及时均匀振捣密实，每点的振捣时间以表面泛浆或不冒大气泡为准，且不超过30s，避免过振。在振捣混凝土过程中，加强检查模板支撑的稳定性和接缝的密合情况，以防漏浆。混凝土浇筑完后，仔细将混凝土表面压实抹平，抹面时严禁洒水。(5)混凝土养护当混凝土强度满足拆模要求，且芯部混凝土与表层混凝土之间的温差、表层混凝土与环境之间的温差均不大于20时方可拆模。拆模后，迅速采用塑料布或帆布对混凝土进行22、后期养护。(6)混凝土表面致密性、减少气泡混凝土在拌和过程会引入大量空气，浇筑成型阶段可以排出大量气体，混凝土表面会滞留一定的气泡；在混凝土配合比设计中，在确保一定的胶凝材料总量，减少用水量，合理选择最佳砂率，使混凝土拌和物获得最大流动性和良好粘聚性及保水性，使粗集料均匀悬浮在水泥砂浆中，保证混凝土拌和物不分层、不离析，最终形成均匀致密的混凝土表面。7.2.2 钢筋工程(1)每批钢筋，附有批号、炉罐号、出厂合格证，以及有关材质、力学性能试验资料等质量证明资料。(2)到工地的每批钢筋按规范要求进行抽样试验，所有试验符合有关标准的规定。钢筋按不同品种、等级、牌号、规格及生产厂家分批验收，分别堆放。23、(3)钢筋的绑扎安装，根据设计图纸规定的钢号、直径、根数间距准确无误，钢筋绑扎牢固不松动，钢筋表面无油污、老锈等。(4)施工前钢筋施工员必须对施工顺序、操作方法和要求向操作人员详细交底，施工过程中对钢筋规格、数量、位置随时进行复核检查。要特别注意一些较复杂部位的钢筋位置、数量及规格。(5)弯曲变形的钢筋须矫正后才能使用，钢筋的保护层厚度依设计图纸规定（净4cm）进行。同一截面钢筋的接头数量符合规范的要求，严格控制插筋位置，避免发生钢筋位移及规格与设计图纸不符。(6)钢筋的焊接加工，在各分部钢筋正式施焊前，按规格要求焊工进行试样焊接，试件应力学试验合格后方可正式进行工程焊接，施焊后从每批成品中切24、取6个试件，3个进行拉伸试验，3个进行弯曲试验（在同一班内同一焊工按同一参数完成的300个同类型接头作为一批）。(7)各种钢筋的焊接接头形式必须符合焊接工艺和质量验收规范要求，焊接试验必须试验，合格后方可使用。接头位置相互错开，在同一截面内受压区50%，受拉区25%，同一根钢筋不得有二个接头，接头位置不可设置在最大弯矩处，且离开变起点10d。7.2.3模板工程(1)模板接缝采用先进可靠的技术工艺，确保接缝满足外观质量要求和混凝土耐久性需要。加强模板的维修与保养，拆摸后及时清理、整修、涂刷脱模剂。(2)必须保证模板的刚度和稳定性，在混凝土侧压力作用下不允许变形，本工程选用大块定型钢模板作为模板，25、以保证混凝土表面的光洁度。(3)模板尽可能减少接缝，接缝位置尽量隐蔽。模板制作的几何尺寸精确、拼缝严密，模板面拼缝高差、宽度不大于1mm，模板间接缝高差及宽度不大于2mm。(4)模板的拼缝宜填实后打磨平整、严密。使接缝平顺；模板的支撑必须牢固、严密，以免发生跑浆和漏浆。(5)模板具有足够的强度、刚度和稳定性，能可靠地承受施工过程所产生的荷载。在模板施工前，必须先进行模板及支撑系统的配置设计，绘出模板排列图。翻样与技术员必须对模板支承、排列、施工顺序拆装方法向班组人员作详细交底。对运到现场的模板及配件按数量、规定逐次清点及检查，不符合质量要求的不得使用。(6)模板结构绝对保证工程结构和构件各部位26、的形状、尺寸相互位置的正确，混凝土施工前认真复核轴线标高、几何尺寸。模板拼缝要紧密，不得漏浆，当拼缝1mm时，采用专用嵌缝条进行嵌实。(7)模板安装前，认真清理干净，附着的水泥浆、铁钉等杂物。并涂刷好脱模剂。对加工的模板事先在地面上进行预拼，以校核平面尺寸和平整度，并检查模板的连结节点，全部合格后方可使用。(8)施工过程中加强对模板的使用、维修管理，以确保模板周转使用的质量，模板拆除后，必须及时清理，要铲除浇筑混凝土时留于模板表面的残浆，清理和整修后的模板表面必须均匀涂满隔离剂。7.2.4原材料控制(1)为保证清水混凝土要求拆模后颜色均匀一致，我们确定原材料为同产地、同厂商、同规格、同品质、同27、颜色的“五同”原则。(2)水泥、掺和料、骨料等原材料品质、颜色的波动必定造成硬化混凝土颜色的差异，同厂商、同规格、同品种不一定同颜色，比如，由于燃烧条件不同以及粉煤灰的组成、细度、含水量等变化，粉煤灰的颜色可以从乳白色变到灰黑色；而品质有差异如骨料级配对硬化混凝土的色泽也有相当的影响。清水混凝土用原材料的“五同”原则是必要条件。原材料供应商从其生产工艺现先进性、原料质量稳定性、保供能力等全方位验证确定，最终确保原材料品质、颜色的一致性。(3)试验发现，不同的胶凝材料用量和不同水胶比的混凝土，其外观颜色也是有较大差异的。胶凝材料用量偏少，一方面容易出现泌水，另外因其包裹骨料的浆量不足，混凝土表面28、粗糙，不够细致；胶凝材料用量偏高，混凝土表面颜色暗淡；在胶凝材料组份中矿粉掺量过高，拆模后即发现混凝土会产生淡绿色的色差，且随时间的延期造成混凝土色泽的变异；粉煤灰的比重比水泥小，在混凝土振捣时，易于上浮，其掺量也会对混凝土色泽影响；另外，混凝土水胶比的大小，混凝土颜色深浅有变化。因此，严格控制胶凝材料用量、水泥用量、用水量和水胶比，达到混凝土色泽控制最佳。8.施工安全文明保障体系8.1安全生产目标8.1.1 确保获得集团公司安全文明生产先进单位，努力创建标准化工地，争创公路工程“平安工地”。8.1.2 重伤及以上生产安全事故为零，一般及以上交通事故为零，机电设备事故为零，火灾事故为零，重大管29、线事故为零，无火灾、洪灾事故，无新增职业病患者。工伤事故率3，工伤事故费率1.5。8.2安全生产管理机构项目经理部建立以项目经理为现场安全生产暨文明施工保证体系第一责任人的安全生产领导小组。本体系规定的安全活动由项目经理负责管理职责，项目经理部管理层其他人员负责配合。本项目经理部应确保各级人员理解安全生产、环境保护管理计划并贯彻执行，本项目经理部的任何部门都应对偏离本计划的现象进行抵制，并对安保体系的实施情况进行管理和跟踪。8.2.1 安全文明管理领导小组的职责:(1)安全文明管理领导小组拟定安全文30、明管理目标，制定安全生产及文明施工保证计划，根据计划的要求，落实资源的配置。(2)负责对安全生产保证体系实施过程实施监督与检查。(3)对安全生产暨文明施工保证体系运行过程中，出现不符合要素的要求，施工中出现的安全隐患，制定纠正和预防措施，并对上述措施进行复查。 图8.1 安全保证组织领导小组结构图安全领导小组组 长：赵明攀 (1)高空施工作业必须经专业安全培训，持证上岗作业。工人必须经过体检合格方可从事相应的施工作业。(2)认真学习安全31、培训内容，施工现场人员必须劳保着装，在高处施工作业时必须系紧安全带，穿防滑鞋。(3)遇风力六级以上(含六级)强风和高温、大雨、大雪、大雾等恶劣天气，停止高空露天施工作业。风、雨、雪过后要进行检查。
