室内游泳池、健身中心、台球房、乒乓球室、阅览区、休闲吧*开元名都康体俱乐部经营主体 开元名都康体俱乐部为上海松江开元名都大酒店所有并经营、管理。*开元名都康体俱乐部经营性质 开元名都康体俱乐部为会员制俱乐部。*开元名都康体俱乐部2、经营宗旨 开元名都康体俱乐部将健身、养身、休闲与商务社交活动完美融合，绝对会员制俱乐部，致力于推广至尊级服务之道，引领时尚风潮，感受尊贵温情，享受快乐时光。第二章第二章 会员会员*会员类型 开元名都康体俱乐部设个人会员、家庭会员。*入会资格 为保障健身安全，凡 16 岁以上至 65 岁以下，具备良好的身体素质及品行和财务资信者可申请加入开元名都康体俱乐部。*入会手续 凡欲申请成为开元名都康体俱乐部会员者，须依规定办理入会手续，填写会员申请表，经开元名都康体俱乐部审核批准，并缴付规定的入会费用，即可成为我们尊贵的会员，领取会员卡，享受俱乐部精心设计的多项优惠礼遇。*会员权利及义务 1.拥有开元名3、都康体俱乐部会员卡者，可凭会员卡进入，并享受相关的会员礼遇。2.会员每次可限带 3 位非会员进入开元名都康体俱乐部。3.会员须认同并履行开元名都康体俱乐部的章程和规定。4.会员须对其邀请的客人的行为及消费负全部责任。5.会员须维护开元康体俱乐部在社会上的形象和声誉，不得扰乱正常的经营秩序。*会员卡 1.会员卡仅限本人使用，不得转借他人消费。2.会员卡客人因本人原因未能在有效期内享用本俱乐部服务，本俱乐部不作退款承诺或延期。3.为保障俱乐部整体秩序，会员进入本俱乐部享用各项设施，必须提前出示有效会员卡证明身份。4.会籍期限：会员卡有效期自办卡确认之日起，至会员年限届满为止。5.会员在每次使用开元4、名都康体俱乐部设施时，必须向服务台人员出示会员卡，结账时须在账单上签名，方可享受优惠。酒店将核对签名。第三章第三章 费用费用*会员卡的费用须提前支付。*会员在俱乐部内发生的一切费用均需当场结清。第四章第四章 会员会籍的转让、遗失和续卡会员会籍的转让、遗失和续卡*开元名都康体俱乐部会员的会籍均可转让。会员卡转让时，须提前通知，其受让人须经开元康体俱乐部审核同意。*转让会籍或补办新卡必须办理书面手续，并收制卡费人民币 100 元张。*自申请办卡之日起，无论在任何情况下，概不退费。*会员卡若有遗失，请会员本人及时书面通知开元名都康体俱乐部作永久注销，并补办新卡。否则会员需自行承担由此造成的全部损失。5、*会员卡到期均需要办理注册手续，会员可在有效期届满后继续缴纳会籍费并办理续会手续。第五章第五章 会员资格取消或暂停会员资格取消或暂停 责任事项责任事项 1）会员应6、遵守开元名都康体俱乐部的各项规章制度，享用相关的设施，并遵从工作人员的指示，以防止意外的发生。2）对在开元名都康体俱乐部所发生的如因会员本人疏忽而造成的损失、受伤等意外事项，开元名都康体俱乐部恕不负责任。3）会员须爱护及小心使用本俱乐部的财务及设施，如在使用中损坏及未经同意擅自拿取，本俱乐部保留追究责任的权利。4）本俱乐部将免费给会员提供更衣柜供存放衣物使用。更衣柜钥匙牌须自行妥善保管，并于离开时交还予接待处。为确保您及其他会员的财务安全，不可将更衣柜钥匙牌带出本俱乐部，若不慎带出或遗失，须赔偿换锁成本费人民币 50 元只。5）贵重物品请勿带入本俱乐部。会员在本俱乐部须自行负责财务安全，若有遗7、失，俱乐部将不做任何经济赔偿。6）为保持本俱乐部的环境整洁，保证每位客人的健康，只可在指定 区域食用点心及饮料。7）为保证本俱乐部的清洁及每位客人的安全，请不要再干蒸房烘烤衣物、染发、焗油、涂奶、盐等。8）本俱乐部运动场所内禁止吸烟。9）会员对自己介绍来使用开元名都康体俱乐部设施的客人，应对其行为及付费等负有责任。10）会员在开元名都康体俱乐部内，如造成本俱乐部或第三方损害时，应负担相关的法律责任。11）第十一条 来宾如不遵守规定，造成开元名都康体俱乐部及其他会员损失时，则由陪同之会员负全部责任。12）本俱乐部严禁大声喧哗，严禁醉酒客人进入，任何客人均不得携带非法性药物及攻击性武器入内。13）8、家长请在任何时候看管好您的孩子，本俱乐部拒绝儿童单独进 入，且不负看管儿童之责任。附则附则*如因政府法律及相关法规或外因等客观情况的变化，经上海松江开元名都大酒店管理当局批准，俱乐部有权对各项目进行改建、修缮、新增，会员不得有异议。*如因营运需要有所变更时，经上海松江开元名都大酒店管理当局批准，俱乐部有权修改本会则，保留对各项收费标准调整的权利。调整的各项收费及任何规定细则将张贴于俱乐部接待台，提前通知所有更 Outside,Tennis Court,Reading Area and Bar.CLAUSE4 Owned&managed by Songjiang New Century Grand Hotel Shanghai.CLAUSE5 The NCG11、CLUB opens to members only.CLAUSE6 Lift your spirits and recharge yourself for life at our state-of-the-art recreation club.It offers an unprecedented array of outdoor and indoor sports and leisure activities,imparts you a feeling of being treated royally.CHAPTER II MEMBERSHIP CLAUSE1 Membership typ12、e:NCGCLUB has individual and family membership.CLAUSE2 Qualification required for membership:between 16 and 65 years of age,well behaved and with good health condition and financial standing.CLAUSE3 Procedure for membership application:If you wish to apply for the NCGCLUB membership you must go thro13、ugh the proper formalities as required by filling in the“Member Application Form”.Once the application has been approved by the NCGCLUB,the required entrance fee and deposit has been paid,you will be a member of the club.CLAUSE4 Members right&obligations:1.Club card holders are authorized to enter t14、he NCGCLUB by showing the card and are entitled to enjoy all members rights.2.Every member can take up to 3 non-member guests into the NCGCLUB each time.3.Members should honor and obey the rules®ulation of the NCGCLUB.4.Members should respect the NCGCLUB image and reputation,and shall not to dist15、urb the business of the club.5.Members shall be fully responsible for the behavior and consumption of his or her non-member companion.CLAUSE5 Membership card:1.The membership card can only be used by the holder and is not transferable.2.The NCGCLUB makes no promise for refund or membership duration 16、extension,in case of no access to the club services in valid duration due to personal reasons.(The NCGCLUB staffs are not authorized to make oral promise of refunding)3.Members should show the card at the entrance to the NCNGLUB so as to ensure a good operation order.4.Duration:from the day that the17、 application is approved till the end of the membership duration.5.Members should show the card every time when they use the NCGCLUB facilities and sign on the bills when they check out.The hotel will verify the signature.(Signature sample will be recognized the first time the member signs the bill)18、CHAPTER III MEMBERSHIP EXPENSES CLAUSE1 The membership fee shall be paid in advance.CLAUSE2 All expenses in the club shall be paid in RMB on the spot.CHAPTER IV RMEMBERSHIP TRANSFER&EXTENSION CLAUSE1 Memberships of the NCGCLUB are transferable.The club shall be notified of the membership transfer in19、 advance and qualification of the transferee shall be verified and approved by the NCGCLUB.CLAUSE2 Membership transfer and issuance of new card in case of loss shall be applied in writing,and RMB100will be charged for each card.CLAUSE3 There is no refund in any cases,once the application is submitte20、d.CLAUSE4 If the club card is lost,the member should immediately notify the NCGCLUB in writing to cancel the card permanently and apply for a new card.Otherwise,the card holder shall have to bear all possible loss and damage.CLAUSE5 Members can renew the membership by paying membership fee and going21、 through extension formalities when the cards expire.CHAPTER V MEMBERSHIP CANCELLATION&SUSPENSION When any of the following situations occurs,the NCGCLUB has the right to cancel or suspend the membership and prohibit the member from using the Club facilities.1.Seriously violate the rules and regulat22、ions of the NCGCLUB.2.Membership fee or any other fees are overdue more than once month.3.Violate the laws and seriously hurt the NCGCLUB image and benefits.4.If the member transfers or lends the membership card to a third party without the NCGCLUBS approval,the club will issue a notification of mem23、bership cancellation or suspension without any refund.CHAPTER VI CLAUSE1 Members should obey the rules®ulations of the NCGCLUB and follow the instructions to avoid accidents.CLAUSE2 The NCGCLUB shall not be liable for the occurrence of loss,injury,and any accident in the club due to personal negl24、igence of members.CLAUSE3 Members shall cherish and carefully use the facilities of the NCGCLUB.The NCGCLUB is entitled to claim on damage or taking away without permission.CLAUSE4 The NCGCLUB offers free use of lockers to members.Keys to lockers shall be carefully kept and returned to the reception25、 desk when check out.Keys should not be taken away from the club for the sake of safety of members belongings.RMB50 Yuan will be charged for each lock changing if keys are taken-away or lost.CLAUSE5 Valuables shall not be brought to the NCGCLOB.Members shall be responsible for safety to belongings i26、n the club.The NCGCLOB will not make any compensation for loss.CLAUSE6 In order to ensure clean environment condition and good health of all guests,members can only take desserts and drink in appointed areas.CLAUSE7 In order to ensure clean environment condition and good health of all guests,clothes27、 drying and use of dye,cream,milk and salt,for hair and skin care in Sauna rooms are not allowed.CLAUSE8 Smoking isnt allowed in all sports areas in the NCGCLOB.CLAUSE9 Members shall be accountable for the behavior and payment of charges of the companion brought in by the member to use the facilitie28、s.CLAUSE10 Members shall be accountable and legally responsible for any injury of a third party that caused by the member.CLAUSE11 Members shall be accountable for any loss and damage on the NCGCLOB or other member caused by their companions improper behaviors.CLAUSE12 Guests should not make big noi29、se in the NCGCLOB,and no drunken guests are allowed in.Any guest should not bring forbidden drugs and weapons into the NCGCLOB.CLAUSE13 Parents should take care of your children anytime.Children are not allowed into the NCGCLOB without accompanying parents,and the club is not accountable for taking 30、care of children.SUPPLEMENTARY PRINCIPES CLAUSE1 With the Songjiang New Century Grand Hotel Shanghai managements approval,the NCGCLOB shall have the right to renovate,maintain and add new facilities,if laws and regulations or external factors change.Members shall be understandable and cooperative.CL31、AUSE2 With the Songjiang New Century Grand Hotel Shanghai managements approval.The club shall have the right to modify the rules®ulations and adjust membership fee and other charges in order to the NCGCLOB reception desk before application without further notice,CLAUSE3 Songjiang New Century Gran32、d hotel Shanghai keeps all right to interpret the NCGCLOB rules®ulations.CLAUSE4 All applicants and holders of membership cards are considered to agree the rules®ulations.The NCGCLOB has the right to differently manage specific violation cases,and the club keeps the right of membership cancellation without any refund.
