proposal based on these Instructions to Bidders,the enclosed Proposal Invitation Letter,PARTS I,II and III,and submit to us as35、 required below.Please direct your Proposal in original plus three(3)copies to the mailing address identified in the Proposal Letter.Please submit your commercial and technical proposals in separate envelopes.Please also provide a softcopy of your Proposal be returned,unopened.There will be no extensions given to the bidders to allow more time in preparation and submittal of the bid.The original of your Proposal should be39、 clearly marked Original.The copies must be identical to the original and may be photocopied from the original after signature.Each package containing material you wish to be considered as part of your proposal must be sealed and marked Sealed Bid.Your failure to mark the package“Sealed Bid”may resu40、lt in the inadvertent opening of the package and may cause your proposal to be rejected.2.0 PREPARATION COSTS Owner will not be responsible for any costs or expenses in preparing and/or submitting your proposal.3.0 GOVERNING TERMS AND CONDITIONS Your proposal must be submitted subject to the Specime41、n Contract as listed of this Request For Proposal by reference hereto.4.0 DEFINITIONS For all purpose42、s of this Request For Proposal document,except as it is otherwise expressly provided herein,or unless the context otherwise requires,the terms defined shall have the meanings assigned to them in this Paragraph.Owner means Contractor means successful bidder.Addendum means the document issued by OWNER43、 to all bidders during the bidding period,containing additional information or corrections made by OWNER to the Request For Proposal document previously issued.Bidder means the person,corporation,partnership,joint venture or other organization that has submitted a proposal to OWNER for performance o44、f the Work.Successful Bidder means the bidder whose proposal has been accepted by OWNER in writing,for the intent purpose of awarding contract.Contract means the contractual document signed by the successful bidder and Owner in the form as defined in this Request For Proposal document.Request For Pr45、oposal or RFP means the solicitation to bid and includes all Addenda.In this Request for Proposal Document the singular includes the plural and vice versa where the context requires.5.0 ACKNOWLEDGMENT Please complete and return promptly the duplicate of the cover letter and secrecy agreement to this46、 RFP as notice of your intention to submit a proposal by the closing date.If the proposal requested herein is to be made from another of your offices,please advise us immediately and have your office submitting the proposal return the signed duplicate cover letter.6.0 WRITTEN PROPOSALS All proposals47、 must be submitted in writing.Wire or telephone proposals will not be accepted.A full and complete version of Proposal must be submitted in both Chinese and English.7.0 RIGHT OF ACCEPTANCE We reserve the right to accept other than the lowest proposal and to accept or reject any proposal in whole or 48、in part,or to reject all proposals with or without notice or reasons,and if no proposal is accepted,to abandon the Work or to have the Work performed in such other manner as we may elect.Partial or incomplete proposals will not be accepted.You will be informed whether or not your proposal was succes49、sful.8.0 BIDDERS MODIFICATION AND WITHDRAWAL OF PROPOSAL You may,without prejudice to yourself,modify or withdraw your proposal by written request,provided that the request is received by us prior to the due date and time at the address to which your proposal was to be submitted.Following withdrawal50、 of your proposal,you may submit a new proposal provided such new proposal is received prior to the due date.9.0 OWNERS POLICY In accordance with our policy,all qualified bidders are entitled to receive equal opportunities to secure our business.The offering or receiving of gifts,entertainment,payme51、nts,loans or other favors for the purpose of being placed on a bidders list,obtaining a contract,or favorable treatment under a contract,is prohibited.Furthermore,it is our policy that in the event a bidder or contractor is found to have offered or given a gratuity to obtain a contract or favorable 52、treatment thereunder,the company involved will be refused further bid considerations by all our entities.We will also obtain those remedies available under law and the contract,including,but not limited to,termination for default.Your attention is invited specifically to those articles in PART III G53、eneral Terms of the Specimen Contract relating to Termination for Default and Gratuities.10.PRICING CONDITIONS 10.1 Pricing,stated in Chinese RMB,must be in accordance with the provisions of Part II,Commercial Terms.10.2 Proposal validity-period is requested for sixty(60)days after the closing date 54、of this RFP.10.3 All pricing must be firm for the duration of this Contract.10.4 The quoted price(s)must include all costs to you for materials,labor,equipment,testing and any and all items of expense,fees,taxes,duties,overhead and profit for your full and complete performance of the Work as set for55、th herein.The quoted price(s)must include all costs in association with overtime and double shifts in order to meet schedule needs.10.5 All pricing information requested in this RFP must be provided.Failure to comply with this request may result in a rejection of any proposal.We reserve the right to56、 request and have you furnish any accounting breakdown of all contract prices.This request may be made after contract price has been finalized with the selected contractor.10.6 If you are the selected contractor you will be responsible for acquiring any and all licenses(applicable contractors licens57、e(s),business license(s),etc.)and permits,excluding building permits,required to perform the Work.10.7 You are advised that quantities reflected on the drawings are for informational purposes only.10.8 Upon award we will issue six sets of drawings for the selected contractors use in obtaining all re58、quired prints.No additional construction drawings will be issued by us for the selected contractors use in performance of the Work 10.9 Proposals for this Work are being invited from other bidders.10.11 n the case of any discrepancy between words and figures,the words shall prevail.In the case of er59、rors in addition or extension,the unit rates quoted shall prevail.10.12 You are advised that the selected contractors personnel,including subcontractors,will be required to attend a safety orientation meeting.(This includes the entire labor force and all new hires).The meeting will last approximatel60、y one hour.All costs are to be included in your quoted prices.10.13 In addition,the selected contractor will be expected to conduct weekly gang box safety meetings attended by all its craft employees.The meetings will last approximately one-half hour.11.INSURANCE 11.1 The selected contractor must,as61、 a minimum requirement,provide the insurance coverage listed under the Insurance Article of PART III,General Terms hereof,and furnish a waiver of subrogation in favor of Owner.11.2 You must complete and submit the insurance data requested in the Proposal Forms,as verification of possession of the ab62、ove-required insurance provisions.12.SCHEDULE Detailed scheduling requirements must be as described in the Article entitled SCHEDULE of PART I,Scope of Work.13.EXCEPTIONS AND DEVIATIONS 13.1 No qualifications to any requirement,provision,specification or stipulation of the RFP document shall be made63、 by you.Failure to comply with this condition may result in a rejection of any proposal.13.2 The submission of a proposal indicates acceptance by you of all conditions contained herein.13.3 We are not responsible for identifying or resolving,during proposal review,any deviations that may be containe64、d in your proposal.If you are awarded the contract,the only deviations recognized will be those mutually agreed upon and written into the contract.14.JOBSITE INSPECTION 14.1 You must inspect the site for familiarization with every phase of the Work and all existing and controlling conditions and lim65、itations.The submittal of a proposal will be considered evidence that you have inspected the site and are aware of all the conditions that may affect the Work and your proposal,whether or not such conditions are referred to in the RFP.14.2 You are required to attend a bid explanation meeting and sit66、e visit in order to become acquainted with the Scope of Work and site conditions prior to submitting your proposal.The bid explanation meeting and site inspection date and place will be informed late.15.DISCREPANCIES AND OMISSIONS/ADDENDA Should you find discrepancies in or omissions from the RFP do67、cuments,or should their intent or meaning appear unclear or ambiguous,you must notify the contracts administrator named in the proposal letter requesting resolution.Replies to such notices may be made in the form of Addenda to the RFP documents which will be issued simultaneously to all invited bidd68、ers.You must acknowledge receipt of all Addenda in your proposal letter.We will not be bound by,and you must not rely on,any oral interpretations or clarifications of the RFP documents.16.CONFIDENTIALITY AGREEMENT Where applicable,you may be required to sign a confidentiality agreement for extra sen69、sitive information provided to you.17.RETURN OF DOCUMENTS In the event you decide not to submit a proposal,you are required to return all RFP documents to us.18.LOCAL LABOR AND SERVICES To the fullest extent possible,you must employ CHINESE LABOR AND SERVICES.Chinese contractors and suppliers of mat70、erials are to be identified in your proposal.19.SIGNATURE REQUIREMENTS The proposal must be signed by an officer duly authorized by your companys legal representatives.If your company is a corporation,the proposal must be signed in its name and on its behalf and under seal by a duly authorized signi71、ng officer of the corporation and must be accompanied by a certified copy of a resolution of the corporation authorizing such execution.The office held by the signing officer must be shown.If you submit a proposal as a partnership or joint venture,you must submit youre your proposal a Power of Attor72、ney executed by all of the general partners or members of the joint venture designating and appointing one of the general partners or members of the joint venture as a Management Sponsor,and authorizing the Management Sponsor to sign the proposal on your behalf,to act for and bind you in all matters73、 relating to the proposal and,in particular,to agree that each partner or member of the joint venture you are jointly and severally liable for any and all of the duties and obligations assumed by you under the proposal and the contract,if awarded.The proposal must be signed on behalf of the partners74、hip or joint venture in its legal name by the Management Sponsor.When requested by us,satisfactory evidence of the authority of any signatory to sign on behalf of your company must be furnished.We will only LICENSES Bidder will submit with its bid the Certificate of License number which grants authority to work as a contractor in the Peoples Republic of China and/or Municipality where the work is to be pe76、rformed.If Bidder does not now have such license or certificate,a copy of the application for same will be submitted with an estimate of time required to obtain it.21.TECHNICAL AND OTHER DATA REQUIREMENTS You must submit the technical and other data as requested.22.GENERAL NOTES 22.1 The proposal mu77、st include the RFP Number on your proposal and other written communications.22.2 Bidder Executive:_ Title:_ Signature:_ CONTRACT DAILY REPORT The Contracto82、r Daily Report is to be filled out by the Contractor,reviewed by Company and maintained by the Contracts Administrator.This report requires the Contractor to report manpower,major equipment on site,activities of the day,and any unusual occurrences.The Contracts Administrator or superintendent must r83、eview this data daily and concur with the Contractors report or rebut in writing.INTRODUCTION The first section contains general information to identifying the contractor,date,shift,work,location,etc.This section must always be completed.The second section is for listing the Contractors manpower and84、 major construction equipment.All units on site shall be recorded in the count including those units of equipment not working on contract work(such as when the Contractor is utilizing units of equipment for its own benefit and maintenance of its own facilities,etc.)which shall have 0 reported under 85、hours.The third section Description of Work Performed Today should report all major types of work and areas in which work was performed.The fourth section Remarks by Contractor is to record any and all unusual or non-routine events which effect operations,such as interruptions,delays,conflicting ins86、tructions,inclement weather,strikes,accidents,and the like.This may include the receipt of a Contract Site Instruction.It may detail a delay occasioned by Company,another Contractor,or the Contractors own operations.The arrival and departure from site of second tier subcontractors should be noted,as87、 well as major changes in Contractors methods of operating.This section should record the details of materials received from Company or others,problems with temporary facilities,or agreements as to the use of the equipment or services.Where the remarks in this section outline a problem,they should a88、lso state what solution has been proposed or who is responsible for resolving the problem.An absence of any references in the Contractor Daily Report to unusual or non-routine events will indicate that there were no such occurrences.The Contractors responsible representative must sign and date the r89、eport,thus placing his remarks(or silence)on record.The final section of the report is designed for the Contract Administrator or Representative to complete any information missing from the first four sections,indicate any disagreements which Company may have with the Contractors reporting,or to mak90、e comments relevant to the contract.The Contracts Administrator or Representative must sign and date each daily report thereby verifying or rebutting the information supplied in the previous four sections by the Contractor and returning a copy to the Contractor.IMPLEMENTATION The Contractor Daily Re91、port should be completed by the Contractor,for each day(regardless of whether work was performed or not)on the morning following the day reported,and submitted to the appropriate Company Superintendent by noon of that same day.Under no circumstances should daily reports be accumulated and filled out92、 retrospectively.The Contractor Daily Report shall not be used in by Companys Contracts Manager.2.7 All formal correspondence from Contractor will be addressed to Companys authorized representative and delivered to Companys Contracts Manager.2.8 Any correspondence no109、t issued in accordance with this procedure will not have contractual validity and Contractor waives any entitlement,recourse or otherwise under this Contract that may arise as a result of acting Drawings and Specifications Drawings,Specificatio115、ns and other related documents will be transmitted to Contractor under cover of the Company Document Transmittal form.Contractor shall acknowledge receipt of such documents by following requirements provided on the form,signing and returning the duplicate form.The form will be numbered as for letter116、s except that the designator letters L shall be replaced by T and a separate sequence of numbers shall be used.3.1.1 Documents marked other than ISSUED FOR CONSTRUCTION,(IFC),or not marked,shall be considered as example drawings issued For General Information Only.These items will be noted as such u117、nder the Status column on the Company Document Transmittal form.3.1.2 Company shall issue one reproducible and one copy of all drawings over size A3(11 x 17).Drawings size A3(11 x 17)and below and specifications shall be issued in one(1)copy only.One(1)copy of drawings and specifications provided to118、 Contractor shall be maintained by Contractor in his site office and shall be made available for inspection and use by Company upon request.3.1.3 Company may issue an electronic copy of documents for the purposes of Contractor prices and the changes on new or revised IFC documents represent only an increase or decrease in quantities which can be measured in accordance with the Contract base pricing structure,this will not 122、be considered to be a change.Contractor shall immediately proceed with any such work in such circumstances.3.2 Contractors Drawing and Data Submittals When required by the Contract,Contractor shall transmit to Company,technical submittals,shop drawings or samples,including supporting catalog cuts,ma123、nufacturers literature,sketches or drawings,calculations and other pertinent data,in sufficient detail to enable Company to review the information and determine that Contractor clearly understands the requirements of the Contract.Documents shall be transmitted to Company under cover of formal contra124、ct correspondence.3.2.1 Documents and data provided by Contractor under the Contract are subject to Company review prior to Contractors start of procurement and/or production on these particular items as noted in Part I to the Contract.3.2.2 Review of drawings,documents and/or data,etc by Company,sh125、all not absolve Contractor from any responsibilities under this Contract or for the design,construction and erection work as defined in the Scope of Work.3.2.3 The review by Company with or without comments shall not relieve Contractor of any obligations or requirements under the Contract,nor be con126、strued as an authorization of,or consent to,any deviation from the Contract.If Contractor considers that Companys comments constitute a change to the Contract,Contractor shall request a change in accordance with Section 15.2 of Part III,and provide cost calculation in the format of the Contract Pric127、ing Format as per attachment.3.2.4 All drawings and other documents shall be in English and be sized in accordance with Metric standard sizes and shall carry titles to indicated equipment numbers or any other identification number of the portion of work covered on the particular drawing and/or docum128、ent.3.2.5 The revision number shall mark changes or additions to any document,at the point of a revision,and the revision must be reflected in its title block or drawing number by an appropriate revision indication.3.2.6 If a document is superseded when returned to Company,indication of the substitu129、tion must appear in or near the title block,or in the transmittal letter.3.3 As Built Drawings 3.3.1 Part I and Exhibit 2 to the Contract shall state the type of drawings that are to be submitted as As-Built drawings.As-built drawings shall be submitted to Company utilizing a Document Transmittal.3.130、4 Quality Control/Assurance Documentation Formal documentation in connection with the Quality Control and Assurance obligations set forth in Attachment E shall be submitted by Contractor to Company utilizing formal correspondence.3.5 Contractors Requests for Substitution.3.5.1 Contractor may request131、 to substitute a material,article,device,product,fixture,form,type of construction or process called for in the Contract with another item that is equal,or better,in all respects to that so indicated or called for.3.5.2 Such requests shall be submitted under cover of a letter in accordance with Sect132、ion 2.0.3.5.3 Companys review and approval or rejection of such request shall not relieve Contractor of any obligations or responsibilities under the Contract.3.5.4 In all cases the request shall be submitted by Contractor with sufficient duration in advance of the requirement for the substitution i133、n order to allow Company sufficient time to fully evaluate the request.Rejection of the request shall not be a basis for a change to the Contract Price or Schedule.4.0 MATERIALS Contractor shall provide all necessary resources to plan,requisition and receive Company supplied materials in accordance 134、with Attachment H,Company/Owner Furnished Equipment and Material Issuing Procedure.5.0 REPORTS 5.1 Daily Reports Each working day,before 10:00 AM Contractor shall submit a Contractors Daily Activity Report for the previous day.This report shall be submitted without a cover letter or other form of tr135、ansmittal.5.2 Weekly Reports Shall be in form and format approved by Company.This report shall be submitted under cover of a letter in accordance with Section 2.0 of this Attachment.5.3 Monthly Reports,Schedules and Schedule updates Shall be in form and format approved by Company.These reports shall136、 be submitted under cover of a letter in accordance with Section 2.0 of this Attachment.5.4 Other Reports Contractor shall submit other reports as required by this Contract.Such reports shall include,but not be limited to:5.4.1 Safety Reports 5.4.2 Safety Incident Reports 5.4.3 Damage Reports 6.0 CO137、NTRACT SITE INSTRUCTIONS(CSI)In accordance with Article 15 of Part III to the Contract,Section 15.4,Company will issue to Contractor Contract Site Instructions(CSIs)defining minor changes to the Work,that are not covered by AFC drawings and for which finalized pricing has not been agreed.The CSI wil138、l define the payment basis.Contractor shall proceed with the work(unless instructed otherwise on the CSI)and within ten(10)working days,shall submit to Company the cost,in the format of a Contract Pricing Format as per attachment,and schedule effects of the CSI with all requisite pricing details.6.1139、 CSIs will be numbered sequentially utilizing specific Contract prefix(i.e.CXXX-CSI-001 through CXXX-CSI-999).6.2 Contractor shall submit,by 10:00 AM,a daily time sheet covering the previous day for approval for any CSI that is issued on an open-ended time and material basis.6.3 Company will periodi140、cally consolidate the agreed costs of CSIs into Contract Modifications.The CSI is not an authority for payment.It is,however,the basis for preparation of a Contract modification,which is the only authority for payment.6.4 Generally CSIs are for low value(under$5,000)works.Works of a higher value wil141、l be covered by a Contract Modification and will not be performed until the modification is issued in accordance with Article 15.0 set forth in Part III to the Contract.7.0 REQUESTS FOR INFORMATION(RFI)In the event that Contractor determines that some portion of a drawing,specification or the contra142、ct document requires clarification or interpretation by Company,Contractor shall initiate and submit a request for information(RFI).7.1 Utilizing the approved RFI form(attached hereto),Contractor shall clearly and concisely set forth the following information:7.1.1 Contract number,RFI number,referen143、ced documents.RFIs will be consecutively numbered beginning with CXXX-RFI-001.7.1.2 The issue for which clarification is sought and why a response is needed from Company.7.1.3 Contractors interpretation or understanding of the Contract requirements together with reasons therefore and any proposed re144、solutions.7.2 Contractor acknowledges that this is a complex Project and it is anticipated that Contractor will issue RFIs to Company throughout the life of the Contract to question and or seek clarification of the Work 7.3 After completion of the form,Contractor submits the RFI to Companys Field En145、gineer with a copy to the Contracts Administrator.7.4 Company will review all RFIs to determine whether or not they are RFIs within the meaning set forth in this Attachment.If Company determines that the RFI is inappropriate it shall be returned within five(5)working days of receipt,without response146、 except to state one of the following:7.4.1 Information to resolve the request exists in the Contract.7.4.2 Information to resolve the request exists on an issued drawing.7.4.3 Information to resolve the request exists in an issued Specification or Document.7.4.4 The use of the RFI form is incorrect147、 and the subject should be covered by a letter,transmittal or other as defined in this Attachment.RFIs returned for any of the foregoing reasons shall be clearly shown in Companys RFI register as“Not Applicable”meaning that they constitute incorrect use of the RFI procedure.7.5 Company shall respond148、 to RFIs within five(5)working days of receipt by Company,unless Company determines that a longer period is required to fully review and respond.In this case,Contractor will be advised of the longer period required within five(5)working days of initial receipt.If Contractor submits a RFI on an activ149、ity with five(5)working days or less of float on the agreed Contract Schedule,Contractor shall not be entitled to any time extension due to the time taken for Company to respond to the RFI.7.6 Companys Field Engineer shall register when RFIs are returned to Contractor.The RFI register shall be revie150、wed at each progress meeting.7.7 Companys response to RFIs are considered to be within the Scope of Work of the Contract,will not change any requirements of the Contract and do not in themselves constitute agreement or authorization to perform a change.In all instances agreement with Contractors int151、erpretation and proposed resolution specifically waives any entitlement that Contractor may have to a change in price or schedule.7.8 If Company considers that the response to an RFI constitutes a change to the Contract Price,Company will issue in parallel a Contract Site Instruction or a Contract M152、odification and this shall be referenced on the RFI response.Only the Contract Site Instruction or contract Modification constitutes authorization to proceed with the RFI response if it is a change.7.9 If Contractor receives a response to an RFI,that Company has not registered as a change,but which 153、Contractor believes constitutes a basis for a Change,Contractor shall not proceed in accordance with the response.Contractor shall immediately notify Company in accordance with 15.2 of Part III and shall not proceed with the Work,which is believed to be changed,without the receipt of a Site Instruct154、ion from Company.8.0 CONTRACT MODIFICATIONS The Contract modification is the only document by which the Contract may be changed or supplemented.It is therefore the only authority for payment of changes and changes may not be invoiced until the appropriate modification has been executed and issued.8.155、1 Company will prepare all Contract Modifications.8.2 Changes covered by a Modification may include:Added or deleted Work Revised drawings or specifications Modified conditions for performance of work or unforeseen field conditions Authorization of overtime Revised requirements for Company or Contra156、ctor furnished materials,equipment or services Schedule revisions Alteration or removal of completed Work 8.3 Both Owner and Contractors authorized representatives shall execute all Contract Modifications.8.4 Contract Modifications shall be numbered consecutively.9.0 MEETINGS 9.1 Major Meetings 9.1.157、1 In addition to formal and informal communications between Company and Contractor,frequent formal routine meetings are necessary throughout the duration of the Contractors performance in order to assure continued communication and record such interfaces for the future.9.1.2 Meetings may be schedule158、d at any time to resolve problems.Major meetings will be scheduled on a regular basis and will be determined as the Contract work develops.Some of the major meetings to be scheduled include:Quality Control&Audit Meetings Safety Meetings Regular progress Review Meetings Contractor Co-ordination Meeti159、ngs Special Meetings Contract Closeout Meetings 9.1.3 Regularly scheduled meetings will generally be held at the Work Site,however,it may occasionally be necessary for Contractor to participate in meetings at other locations,e.g.,Company or Owner Home Office.9.2 Regular Progress Review Meetings 9.2.160、1 Progress review meetings will be held on a regularly scheduled basis,usually weekly.These meetings will provide a forum for review of Contractors operations,assessment of progress and schedule,discussion and resolution of problems facing the Contractor and Company,and coordination of the activitie161、s of all parties concerned.9.2.2 In general these meetings require a minimum of participation to achieve the maximum positive results.9.3 Contractor Co-ordination Meetings Contractor Co-ordination Meetings will be scheduled as required to discuss problems requiring Contractor interfaces and co-ordin162、ation.9.4 Special Meetings 9.4.1 Special meetings will be organized as required to discuss and plan Work and resolve problems,etc.9.4.2 Further,Contractor shall conduct a program of safety motivation and training,including regular safety meetings(tool box meetings)for its personnel in accordance wit163、h Attachment D.Contractor shall minute these meetings and submit to Company for information.9.5 Contract Closeout Meeting At completion of Contractors Work or as soon thereafter as practicable,a Contract Closeout Meeting will be held between Company and Contractor.The purpose of this meeting is to r164、each an agreement on all outstanding items,i.e.Contract Modifications,final payment amounts,etc.9.6 Minutes of meeting Company will prepare minutes of meeting for all meetings addressed above(except toolbox meetings).The Minutes of Meeting will be complete with all significant aspects of the meeting165、 recorded and will be presented to the Contractor for signature as soon as practicable after the meeting.After Declarations required by Part II to the Contract and in the format as per attachment.11.0 SIGNATURE AUTHORITIES 11.1 Contractor shall provide,on the Signature Authorization Form,the names and specimen signatures 168、of those individuals within the Contractors organization authorized to sign documents on behalf of the Contractor.Contractor shall also specify the financial or other limits of authority for each individual.11.2 Contractor shall delegate authority within its organization to Home Office and Field Off169、ice personnel as required for effective performance of the Work.The Signature Authorization Form shall be signed by the Contract Signatory of the Contractor.12.0 STANDBY 12.1 Standby costs will not be reimbursed to Contractor should any of the following criteria be met:Labor and/or equipment can be 170、utilized elsewhere.Idle time is as a result of drawing holds or lack of information which could be avoided or which Contractor failed to give prior notification in accordance with Article 4.0 of Part III.Shutdown of operations due to plant emergency lasting up to three(3)hours,or the first three(3)h171、ours of a shutdown having a longer duration.Time spent on safety related matters such as safety indoctrination,safety meetings,and safety drills or safety infringements.Late issue of work permits by up to one(1)hour.Should period of late issue exceed one(1)hour then standby time will be paid retroac172、tive to the start of normal work time.Contractor has failed to orally notify Fluor within thirty(30)minutes of a standby situation or submit a standby report within two(2)hours of the commencement of standby.12.2 Should a standby situation arise and Contractor cannot utilize resources elsewhere Cont173、ractor shall:Within thirty(30)minutes following rules,regulations and Site conditions apply to Contractor and all Subcontractors under its control.The attachment C is a summary of the site requirements and it will serve as the minimum HSE requirements203、 for the project.Any project safety regulation,which exceeds the minimum standards,established by relevant contractors shall be incorporated in Contractors HSE program.The contents contained herein are not all inclusive.All China National Legal Standards/Regulations and OSHA standards not specifical204、ly referenced in these rules®ulations shall apply when applicable.This material must not be considered to be all inclusive as to the hazards that might be encountered,safe practices that should be performed,or safe conditions that should be maintained during the course of this project.Contractors205、 and sub-contractors will read and understand the full extent of the COMPANYS HSE Management System including Specific HSE Plan,which are available on request from the COMPANY.Project HSE program has been prepared to assist each contractor in satisfying its moral,contractual and legal accident preve206、ntion responsibilities,in such a manner that a safe,efficient operation is assured.For additional advice and assistance each contractor should consult with its safety organization,insurance carrier or similar available source.In the bidding stage,Contractor and all Subcontractors shall take the full207、 consideration of the direct and indirect cost associated with Field Construction,Health,Safety&Environmental and Security.All these considerations shall be based on the full understanding of Contract Attachment C.In the contract execution stage,Contractor and all Subcontractors shall strictly perfo208、rm the project in accordance with Contract Attachment C,COMPANYS HSE Management System and all relevant national laws and regulations to the satisfaction of COMPANY.1.2 Site Location and Access Roads Contractor is required to make all inquiries concerning special public road limitations in the area 209、of the Site with regard to deliveries of Equipment and material.There will be specific gates that Contractor may use and sign posted routes between the gates,the Site entrance and any areas outside the Site.Contractor may have use of the Project Owners roads for the purpose of obtaining access to th210、e Site and carrying materials,Equipment and personnel thereon and shall be responsible for the provision of all associated means of transport.The maximum speed limit on the CFM Project is 20 km/hour unless otherwise indicated.Contractor will be responsible for making good any damage caused at its ow211、n expense or pay all costs and charges in connection therewith.Contractor shall consult with COMPANY as to the suitability and strength of the roads for the type of vehicles which the Contractor,its Subcontractors or suppliers intend to use.Movement of men,vehicles,plant and materials over Project O212、wners roads shall be subject at all times to the control of COMPANY.1.3 Site Layout and Zone Identification Contractor must ensure its and its Subcontractors employees are fully aware of the Site layout,approved routes to and from the Site and specific operational zones thereon.1.4 Record of Key Per213、sonnel-On/Off Site Contractor will notify COMPANY the details of key personnel who are to be contacted in cases of emergency.These details shall include,Site telephone numbers,home telephone numbers and/or home addresses.For the Leave and change of Contractor Key Personnel,Contractor shall notify CO214、MPANY in advance with written document.And no leave and change of Contractor Key Personnel shall occur without the written approval of COMPANY.Key Personnel include Project Manager,Construction Manager,HSE/Security Manager,Full Time HSE Personnel and Medical/First Aid Personnel.1.5 Integrity of Reco215、rds All information requested of and supplied by Contractor to COMPANY,will be treated as confidential and access to such information will be restricted to authorized personnel of COMPANY.Contractor will advise COMPANY of any changes or amendments to information previously supplied.1.6 Utilities Sup216、ply and Regulations a.Electrical Supplies and Regulations i.Temporary Building Power,Lighting and Heating Supply All electrical installations within temporary buildings shall be in accordance with Site Safety Rules/Regulations and relevant National Laws/Regulations,and shall be subject to check by C217、OMPANY.ii.Fuses and Equipment In no circumstances shall Contractor interfere with fuses of electrical equipment belonging to OWNER.Before Contractor plugs in any electrical appliance to any plug socket belonging to OWNER he shall ensure that the appliance is in good condition and is fitted with a su218、itable cable including fully rated and insulated neutral conductor and protective earth conductor.iii Contractors Wiring Contractors distribution system,lighting systems and hut wiring shall be installed in an approved manner and maintained in a satisfactory condition.iv Care of Cables No weight sha219、ll be imposed upon any electric cable and no staging,ladder or similar equipment shall rest against or be attached to it.No splices shall be buried into ground.Cables shall be properly elevated off ground or buried into ground with clear warning signs.Temporary power cables in use by Contractor must220、 be positioned so that they do not cause a tripping hazard.vi Temporary Power Plan and Administration Before the commencement of cable pulling and installation of temporary power system,Contractor must develop and submit the Temporary Power Plan to COMPANY for written approval.Tie-ins with COMPANY e221、xisting power service must be approved by COMPANY in writing and Tie-in Record shall be filed in documentation.Contractor must assign competent/fulltime electrician to administrate and maintain Temporary Power System.b.Water Supply and Drainage Before the commencement of site water supply system ins222、tallation,Contractor must develop and submit the Temporary Water Supply Plan to COMPANY for written approval.Tie-ins with COMPANY existing water system must be approved by COMPANY in writing and Tie-in Record shall be filed in documentation.Excavation,de-watering and test water must be disposed of i223、n a controlled manner.Prior approval from COMPANY must be obtained before discharge commences.Contractor must maintain the drainage/de-watering system on site.1.7 Existing Services Contractor shall take every practicable precaution not to damage existing installations or services whether these are e224、xposed or buried.Contractor shall advise COMPANY at least 24 hours in advance of the movement of any heavy loads of material or equipment within the Site or the laydown area.Should damage occur to existing services during the course of a Contractors operation,the cost of rectifying the damage lies w225、ith the Contractor responsible for causing the damage in the first place.1.8 Contractors Temporary Facilities a)The location and function of all temporary buildings and storage areas must be agreed with COMPANY prior to installation by a written plan.b)All temporary buildings and storage areas must 226、be barricaded or isolated with other areas as different functions or parties.Barricades or isolation materials shall be rigid metal or bricks/concrete.c)On completion of erection,temporary facilities will be inspected by the COMPANYS Site Safety Officer 1.9 Industrial Terminology Contractor must ens227、ure its employees are conversant with terminology associated with the industry particularly those materials/solutions of a hazardous nature.Contractor must have available on Site a Material Safety Date Sheet for each material solution that may be hazardous.This sheet must specify the actions to be t228、aken in an emergency.This sheet shall be in both Chinese and English.1.10 Storage Compounds a)The location of storage compounds must be agreed with COMPANY before storage of materials commences.Such compounds shall be maintained for the storage of the approved materials and for no other purpose.b)Oi229、ls,greases and similar materials must be stored in non-flammable bins or buildings or in a fenced compound remote from other inflammable materials c)All high-pressure gas cylinders shall be stored in racks in an upright position.Acetylene and Oxygen cylinders shall be segregated by a fire partition 230、approved by COMPANY.Full cylinders shall be stored separately from empty cylinders.All cylinders shall be stored with metal protective caps and anti-shock rings fully installed into place.d)No Smoking and“Fire Caution”signs shall be provided by Contractor and prominently displayed in areas where fla231、mmable materials are stored.Additionally,Contractor shall provide and maintain suitable fire extinguishers in such areas.1.11 Materials Issue and Transportation Bulk materials stores must be positioned away from the work face but within the Site or Contractors designated laydown area or the laydown 232、area.A limited number of small fittings may be located adjacent to the work face in small warehousing facilities with COMPANY approval.1.12 Restriction on Movement within OWNER Property Movement of personnel/Equipment outside the approved operation routes and areas and by means other than via author233、ized transport is prohibited.Contractor must ensure its employees and its Subcontractors employees are fully aware of this restriction.1.13 Obstruction of Plant Facilities Contractor must ensure that its own or its Subcontractors plant or Equipment does not obstruct access or areas around any operat234、ional plant area.1.14 Tipping a)No stockpiling of excavated material will be allowed on the Site or within the Project limits without COMPANYS approval.b)Contaminated waste or other material subject to statutory control must be deposited in a properly approved container and disposed of in a controll235、ed depository outside the Project limits approved by COMPANY.c)Without the permission of COMPANY,Contractor is prohibited from stacking or dumping any industrial or living rubbish/waste/debris in project boundaries.Contractor shall be responsible for transportation and disposal of all industrial or 236、living rubbish/waste/debris outside of Project Boundaries in accordance with relevant National Laws and Regulations.1.15 Cameras Contractors employees and the employees of its Subcontractors must not,under any circumstances,bring cameras onto the Site or the laydown area or use a camera on Site with237、out the written approval of COMPANY or OWNER.1.16 Pre-Qualification of Subcontractors Contractor must ensure that its pre-qualification procedures for its Subcontractors fully take into account the ability of the proposed Subcontractor to comply with COMPANYS rules and regulations and applicable cod238、es of practice,particularly those related to safety,health,welfare and security.Contractors cannot sub-contract work without the express permission of Company in writing.1.17 Documentation a)Certification of Competence Contractor shall ensure that certification/qualification of its personnel and the239、 personnel of its Subcontractors are in accordance with statutory requirements and applicable laws.b)Records Records pertaining to the qualifications of Contractors personnel shall be maintained and be made available to OWNER or COMPANY.On request the records shall be readily accessible.1.18 Radios 240、on Site Hand held radios may be permitted.Approval for use of radio is to be obtained through COMPANY.Contractor shall obtain any necessary license to operate radios on the Site in compliance with PRC laws.2.0 HEALTH,SAFETY AND ENVIRONMENT(HSE)2.1 Objectives Contractor and its Subcontractors must co241、mply with COMPANY standing orders,COMPANY HSE regulations and all reasonably practicable measures towards the attainment of the project objective,which is to raise the safety awareness of every individual on Site or the laydown area in order to achieve zero accidents.Contractor and its Subcontractor242、s shall comply with all Chinese government regulations including labor and employment regulations with regard to medical,first aid,safety and training requirements.2.2 HSE Policy/Responsibilities a)Contractor and its Subcontractors shall submit their HSE policies for approval prior to commencement o243、f work on Site.Contractor and its Subcontractors must ensure their COMPANY policies for safety,health and environment are maintained in a practical and up to date format and that individual responsibilities are clearly defined,fully understood by their employees and properly executed.Policies shall 244、show how the commitment to safety of all employees will be achieved and maintained.b)COMPANY executes Non Tolerance Compliance Policy on HSE.COMPANY will stop any work which puts at risk the health and safety of any person,or could cause damage to materials,plant or Equipment and shall require the C245、ontractor concerned to carry out immediate remedial measures at its own expense.For any company,group and individual who breach HSE policy,program and goals,COMPANY holds the rights to back-charge management fees,suspend/cancel contract,remove from site and other further actions.c)Where Contractors 246、work includes the undertaking of hazardous Operations,it will submit a written statement in respect of carrying out that work to COMPANY for review before commencing.These operations will include,but not be limited to,the following:-Excavation-Operation of Explosive Tools-The Erection of Structural 247、Frames and Panels-Abrasive Blasting-Operations in Confined Spaces(vessels,tanks,pipes,manholes,sewers,excavations)-Handling and Usage of Toxic and Hazardous Substances Including Submission of MSDS(Material Safety Data Sheet)-Any lifting operation in which the load to be lifted is more than 85%of the248、 crane manufacturers specified safe working load at the proposed radius of operation-All lifting operations where the load exceeds 30 tons.-Amounts of Scaffolding Operations-Radiography Operation-Operations involves coordination with other ongoing operations,over,under or nearby-Other operations or 249、systematic activities identified with the needs of special safety prevention measures d)Contractor shall submit a HSE plan for the Work.This shall include details of the risk assessments carried out and the proposed control procedures and working methods to ensure that the risks identified are safel250、y dealt with.The plan shall also set out Contractors proposals for ensuring that those affected are provided with information,instruction and training concerning the risk assessment.e)Written or implied approval by COMPANY of any method statement,safety plan or other requirement of this Attachment s251、ubmitted to COMPANY shall not relieve the Contractor of its responsibilities to comply with all statutory legislation with regard to the health and safety of its employees and all other persons who may be affected by its acts or omissions whilst undertaking the Works.2.3 HSE Organization/Coordinatio252、n a)Contractor shall employ a full time qualified and experienced HSE Officer for any workforce up to 50 people(this is inclusive of its Subcontractors).Thereafter,a HSE officer/workforce ratio of 1 to 50 will apply.For a workforce less than 50 people Contractor shall nominate a full time qualified 253、and experienced HSE Officer.Contractor HSE officers will work under the supervision of COMPANYS HSE supervisor.b)Contractor shall display in its Site office all statutory safety notices and COMPANY HSE Plan.Contractor shall display its project specific HSE Plan in meal rooms and Site offices.Contrac254、tor shall provide COMPANY with copies of both plans prior to commencing Work.c)Contractor and its Subcontractors shall comply with and participate effectively in the prescribed arrangement for HSE co-ordination on the Project.d)Contractors Project Manager together with Contractors HSE Manager shall 255、attend the Weekly HSE Management Meeting,which is organized by COMPANY HSE Department.This responsibility shall not be delegated unless written agreement has been received from COMPANY.This meeting shall form the basis for the project HSE Plan developments and identify incident trends and remedial a256、ctions that may be required.e)Contractors Project Manager shall attend the Monthly Management HSE Committee Meeting,which is organized by COMPANY Senior Management.This responsibility shall not be delegated unless written agreement has been received from COMPANY.This meeting shall form the basis for257、 the project HSE Plan developments and identify incident trends and remedial actions that may be required.f)Contractor and its Subcontractors shall hold toolbox meetings at daily intervals wherein supervisors will address the workforce on HSE related matters.Topics discussed shall be pre-published t258、o COMPANY.Records shall also be retained of all attendees together with their signatures.g)Al employees of Contractor and its Subcontractors shall attend the Site Level Toolbox Meeting,which is organized by COMPANY HSE Department.This meeting will be organized on weekly or monthly basis as the face-259、to-face communication between COMPANY HSE Management and site employees.h)Contractor and its Subcontractors are encouraged to introduce safety incentives to encourage safety awareness of their workforce.i)Contractor and its Subcontractors must undertake regular inspections and formal HSE audits both260、 independently and in consultation with COMPANY as and when they may be required.j)Contractor and its Subcontractors must notify COMPANY of any visits by Governmental HSE Officers and the serving of any prosecution,prohibition or improvement notice,or warning letter.In addition,full details of the c261、ircumstances relating to the issue and implications of these and the remedial action required must be fully discussed with COMPANY.2.4 Promotion/Stimulation Contractor and its Subcontractors are encouraged to introduce HSE incentive and disciplinary program.And execute it as the important part of HS262、E Execution Plan and improve the performance and awareness of employees.Contractor shall provide and install the following HSE Information Facilities,but not limited to:-HSE Signs:Prohibition,Mandatory,Restriction,Hazard Warning,Danger and Attention etc.-HSE Bulletin-HSE Slogans,Banners-Work Hours B263、oard,Incentive/Discipline Bulletin Signs shall be removed when the hazard no longer exists or make proper arrangement to ensure the effectiveness of signs.2.5 Reporting of Incidents/Near Misses All incidents/near misses resulting in or having potential for injury,damage or loss must be reported to C264、OMPANY and,where an injury-however slight-occurs,it must be recorded in both Contractors and COMPANYS accident books.Any incident or dangerous occurrence involving plant,workers,Equipment must be reported immediately to COMPANYS HSE Officer,both verbally and in writing.Written reports must detail th265、e nature of the incident,the time and date of the occurrenceand the precise location,as well as other pertinent factors that may have contributed to the occurrence.All statutory notices sent to the enforcing authorities concerning the occurrence shall be copied to COMPANY.Copies of reports of all in266、cidents reported to the governmental safety authorities shall also be sent to COMPANY.Full details of any witnesses must also be obtained and any breach of statute or permit to work arrangements must be recorded.Where personal injury has occurred,a medical report must be obtained and included with t267、he written report or submitted immediately afterwards.The senior management representative of the Contractor and of the Subcontractor involved in an accident/near miss will be required to investigate and give a detailed account of the circumstances of the incident.2.6 Investigation and Follow-up Con268、tractor shall submit the official investigation report to COMPANY in 24 hours after the occurrence of Accident/Incident/Near Miss.This report shall at least include the following contents:a)Time,Location and Personnel involved b)Consequences,initial status of Emergency Response and Reporting c)Causa269、l Analysis Direct and Indirect d)Remedial Actions Recommended and Execution Plan e)Witness Records and other related documents and records.Contractor shall strictly complete the follow-up actions and confirm the completion with COMPANY.The end of this step means Investigation and Follow-up completed270、.2.7 HSE Orientation and Training No person will be allowed to commence work on the Site before he has been given COMPANY New Hire HSE Orientation training(free of charge)in COMPANY HSE Training Center.This orientation will include instruction and information on the following:a)Contractors health an271、d safety policy and its implementation b)Safety advice and accident prevention c)Safety responsibilities-individuals,supervisors and management d)The role and function of the safety organization e)The purpose of tool-box and other safety meetings f)Distribution of Contractors safety handbook to empl272、oyees,listing the basic safety rules pertaining to the Site(e.g.personal conduct,safety equipment,housekeeping etc)g)A detailed explanation of Site emergency procedures h)Welfare,the first aid facilities provided,including location and condition of use i)The procedure for reporting injuries and dang273、erous occurrences,including the role of the accident book j)Security arrangements and personal identity badges k)The requirements of Site Rules and Safety Regulations l)Explanation of the work permit procedure,permit to work and confined space procedures.OWNER and COMPANY will require Contractor to 274、provide regular safety training for its employees and Subcontractors employees during the term of the Contract.Topics covered may include working at heights,Site tidiness,work with cranes,excavation,permit to work procedure,confined space work.OWNER and COMPANY may participate in such training by mu275、tual agreement.COMPANY will arrange Site wide training sessions as necessary and Contractor will ensure that its employees and its Subcontractors employees attend when required to do so at no cost to OWNER and/or COMPANY.2.8 Competent Supervision All work shall be carried out under competent experie276、nced and qualified supervision.Supervisors,foremen and others with managerial or supervisory responsibilities must be trained and experienced in the operation for which they are responsible.In addition,they must be fully conversant with all relevant health and safety legislation that applies to the 277、work for which they are responsible and be conversant with appropriate safe methods of work.They must be aware of the Site emergency procedures and their responsibilities thereunder.Where required they will initiate or undertake induction,safety or other training of personnel whom they co-ordinate o278、r control.They must also be familiar with the COMPANYS Site Rules and Safety Regulations and permit to work requirements and confined space procedures in the operation of the Work.2.9 Management HSE Committee The COMPANY will establish a Management HSE Committee.The committee including representativ279、es from Contractors(Project Manager,Construction manager)will meet monthly under the chairmanship of the COMPANYS Project Manager,or its delegate.The secretary will be the COMPANYS Safety Supervisor who with the agreement of the chairman shall convene meetings,take and circulate minutes,and monitor 280、action taken thereon.2.10 Site Emergency Procedures Contractor and its Subcontractors shall comply with the requirements of the Site emergency procedures and such variations or additional requirements as may be imposed on the Work from time to time by the OWNER and/or COMPANY.A copy of the procedure281、s shall be posted in all Site offices and change rooms.a)Alarms Contractor and its Subcontractors must ensure that their employees are fully aware and conversant with the respective alarms associated with the emergency procedures on the Site,also the measures to be taken in the event of an alarm bei282、ng sounded.b)Evacuation Plan Contractor and its Subcontractors must ensure that their employees are fully aware of,and conversant with,the plan for evacuation of the Site should the occasion arise.c)Life Saving Devices and Protective Equipment Contractor must ensure that employees are fully aware of283、 the location of,and are competent to use,life saving devices and protective equipment.Where training is required for the use of specialized equipment,Contractor shall ensure that employees receive the level and standard of training as required.2.11 Site Services and Accommodation COMPANY HSE Orient284、ation is mandatory to all Site Services Personnel of all Modules/Products/Equipment before the entry to site.Contractor must provide necessary HSE training and fulltime supervision to these personnel and ensure the full conformance to COMPANY HSE rules and procedures.No accommodation facilities or p285、ersonnel on site are permitted.Contractor personnel of nightwatch and security must be in fulltime duty,and no sleeping and accommodation on site.2.12 Pressure Vessel and Equipment All pressurized plant will be installed,tested,inspected and maintained by competent persons.Details of all such inspec286、tions,examinations and tests carried out in compliance with legislation will be forwarded to the COMPANY,who shall prohibit the use of such plant in the absence of current certificates.2.13 Equipment Inspection All equipment/tools/vehicles shall be inspected by competent persons designated by Contra287、ctor before the first usage on project site,and after then on monthly basis.All inspection shall be recorded in written and filed for the convenience of COMPANY check.All inspected equipment/tools/vehicles shall be properly tagged with color coding system(waterproof stickers with colors of Red,Blue 288、and Yellow,change monthly in turn)Belts,gears,shafts,pulleys,sprockets,spindles,drums,fly wheels,chains or other reciprocating,rotating or moving parts of all equipment exposed to contact by any person,shall be permanently and soundly guarded.All energy releasing sources such as power supply termina289、tions,compressed air supplies etc.shall be isolated or insulated properly.The following requirements shall be met for Pedestal Drills,Grinders:a)No person shall remove any safety guard device from a grinding or drilling machine unless the equipment is locked out or tagged out of service.b)Grinding a290、nd drilling operations produce airborne projectiles.Operators of such machines shall protect themselves and others in the work vicinity by the use of personal protective equipment,the minimum being hearing and eye/face protection.c)No person shall operate a bench or pedestal drill;grinder in a tempo291、rary work situation until the equipment has been leveled and securely anchored.d)No grinder hall be fitted with any other cutting wheel,eg saw blade,polishing disc or pad,etc than those recommended by the manufacturer.e)Persons operating Pedestal Drills and Grinders must be pre-trained by Contractor292、.2.14 Storage of LPG and Other Hazardous Materials No bulk storage of LPG will be allowed.Proposals for the storage of highly flammable liquids,liquefied petroleum gases and other flammable and hazardous materials will be submitted to COMPANY for approval.Proposals shall include the siting,design,in293、spection and maintenance of the storage facilities and the name of the nominated person who will be responsible therefore.All relevant COMPANY Site instructions and National Laws and Regulations will be complied with.2.15 Rebarwork and Formwork Persons shall not be permitted to work above or adjacen294、t to(where there is a likelihood of them falling or stumbling)vertically protruding reinforcing steel,unless such steel has been adequately guarded to eliminate the potential of impalement.Rebarwork and formwork plans approved by COMPANY shall be available before the commencement of works.2.16 Scaff295、olding and Ladders Scaffolding design and construction will be generally in accordance with the following criteria and to the satisfaction of COMPANY.Bamboo scaffolds/planks,knitted bamboo strips and ladders,will not be used on site.Should further guidance be required it is recommended that Chinese 296、Scaffolding Legislation/Standards will apply.a)General requirements 1).Scaffolds shall be erected in accordance with requirements of this section and shall have strength not less than 2.4 kg/square cm.2).The foundation of scaffolds shall be pre-treated with reinforcement.The footing or anchorage for297、 scaffolds shall be sound,rigid and capable of carrying the maximum intended load without settling or displacement.Unstable objects such as barrels,boxes,loose brick,or concrete blocks,shall not be used to support scaffolds or planks.3).No scaffold shall be erected,moved,dismantled,or altered except298、 under the supervision of competent persons.4).Guardrails and toe-boards shall be installed on all open sides and ends of scaffold platforms.5).Guardrails shall be 5 x 10cm construction grade lumber,48mm(2.753.5mm)pipe(standard scaffolding tube),or the equivalent,90 cm to 1.1 m high,with a mid-rail.299、Supports shall be at intervals not to exceed 2.4 m.Toe-boards shall be a minimum of 10cm in height.Guardrail systems shall be capable of withstanding,without failure,a force of at least 200 pounds(890 N)applied within 2 inches(5.1 cm)of the top edge,in any outward or downward direction,at any point 300、along the top edge.6).Where persons are required to work or pass under the scaffold,scaffolds shall be provided with debris netting between the ledgers and toe-boards on all elevations of the scaffolds.7).Scaffolds and their components shall be capable of supporting without failure at least 4 times 301、the maximum intended load.8).Any scaffold including accessories such as braces,brackets,trusses,screw legs,ladders,etc.damaged or weakened from any cause shall be immediately repaired or replaced.9).All load-carrying timber members of scaffold framing shall be construction grade lumber which satisfi302、es Chinese Legislation/Standards 10).All planking shall be Scaffold Grades,or equivalent,as recognized by approved grading rules for the species of wood used.The maximum permissible spans for 5 x 25 cm or wider planks shall be 1.2 m.11).All planking of platforms shall be overlapped(minimum 0.3 m),or303、 secured from movement.12).An access ladder or equivalent safe access shall be provided.13).Scaffold planks shall extend over their end supports not less than 15 cm nor more than 30 cm.14).The poles,legs,or uprights of scaffolds shall be plumb,and securely and rigidly braced to prevent swaying and d304、isplacement.15).Overhead protection shall be provided for men on a scaffold exposed to overhead hazards.16).Slippery conditions on scaffolds shall be eliminated as soon as possible after they occur.17).No welding,burning,riveting,or open flame work shall be performed on any staging suspended by mean305、s of fiber or synthetic rope.Only treated or protected fiber or synthetic ropes shall be used for or near any work involving the use of corrosive substances or chemicals.18).Wire,synthetic,or fiber rope used for scaffold suspension shall be capable of supporting at least 6 times the rated load.19).T306、he use of lean-to-scaffolds is prohibited.20).Lumber sizes,when used in this subpart,refer to nominal sizes except where otherwise stated.21).In case of work near electrical cable or equipment which has no insulation,employees shall not work closer than the distance specified,for each level of volta307、ge stated below,both horizontal and vertical:Distance of not less than 2.40 meters for electricity from 50 volts upward to 12,000 volts.Distance of not less than 3.00 meters for electricity from 12,000 volts upward to 33,000 volts.Distance of not less than 3.30 meters for electricity from 33,000 vol308、ts upward to 69,000 volts.Distance of not less than 3.90 meters for electricity from 69,000 volts upward to 115,000 volts.Distance of not less than 5.30 meters for electricity from 115,000 volts upward to 230,000 volts.22).Scaffolding shall be erected and maintained by Trained and competent Scaffold309、ing Erectors and Supervision.A scaffolding tag(Red and Green)or similar system will be adopted for all scaffolding to ensure that scaffold inspections are carried out at least weekly by a competent person on behalf of Contractor whose employees are using it.The results of these inspections will be e310、ntered in a scaffolding register.COMPANY will regularly inspect the registers to ensure they are being kept up to date.A competent person employed by one Contractor may carry out inspections on behalf of other Contractors using a scaffold,by mutual agreement.23).The names and resumes of all appointe311、d competent persons shall be submitted to COMPANY before they commence their duties.COMPANY may refuse to recognize,or withdraw recognition of,a competent person at its discretion.24).The unauthorized modification(e.g.,temporary removal of bracing,toe-boards,etc.)of scaffolds is prohibited.25).All s312、uspended scaffolds shall be anchored to prevent swinging.The suspended support shall be electrically insulated when arc welding is to be performed from the scaffold.Workers on suspended scaffold work platforms must use independent safety lines and safety harnesses/lanyards.26).Only trained operator 313、with official license shall operate all hydraulic platforms.27).All ladders are to be soundly constructed and properly maintained in accordance with governmental legislation and to the satisfaction of COMPANY.28).Ladders are to be registered and tagged.They shall be inspected before and after use an314、d any observed defect remedied at once.29).Portable ladders when in use shall be secured,by a competent person,with rope or ladder clamps to prevent slipping or overbalancing.30).Ladders shall always have a firm footing to prevent the ladder feet from moving unexpectedly.Wooden blocks,off cuts,brick315、s,etc shall not be used to level the feet of any ladder.31).All ladders shall be equipped with approved safety feet.32).Contractor shall ensure that ladder length,angle of placement,measures to secure the ladder and measures to prevent undue swaying,slipping,etc.are suitable for the specific task to316、 be undertaken.33).Portable ladders shall be placed on a substantial base at a 4:1 pitch,have a clear access top and bottom and extend a minimum of one(1)meter above the egress landing.34).Portable metal ladders and wire reinforced ladders(All conductive ladders)shall not be used for any electrical 317、work or where contact with electrical conductors is foreseeable.35).Rungs of portable ladders and fixed ladders should be spaced not less than 25cm,and not more than 40cm.36).Straight Portable Ladders must not be longer than 5 meters.37).The use of job-made ladders is discouraged.38).No more than 1 318、person shall work from the same ladder unless it is specifically designed for 2 people.b)Tube and coupler scaffolds.1).A light duty tube and coupler scaffold shall have all posts,bearers,runners,and bracing of nominal 50 mm O.D.steel tubing.The posts shall be spaced no more than 1.8 m apart by 3.0 m319、 along the length of the scaffold.Other structural metals,when used,must be designed to carry an equivalent load.No dissimilar metals shall be used together.2).A medium duty tube and coupler scaffold shall have all posts,runners,and bracing of nominal 50 mm O.D.steel tubing.Posts spaced not more tha320、n 1.8 m apart by 2.4 m along the length of the scaffold shall have bearers of nominal 60 mm O.D.steel tubing.Posts spaced not more than 1.5 m apart by 2.4m along the length of the scaffold shall have bearers of nominal 50 mmO.D.steel tubing.Other structural metals,when used,must be designed to carry321、 an equivalent load.No dissimilar metals shall be used together.3).A heavy duty tube and coupler scaffold shall have all posts,runners,and bracing of nominal 50 mm O.D.steel tubing with the posts spaced not more than 1.8 x 2.0 m.Other structural metals,when used,must be designed to carry an equivale322、nt load.No dissimilar metals shall be used together.4).Tube and coupler scaffolds shall be limited in heights to 26 meters.Drawings and specifications of all tube and coupler scaffolds above these limitations shall be designed by a qualified engineer competent in this field.5).All tube and coupler s323、caffolds shall be constructed and erected to support a uniformly distributed static working load of 1.5 kN/m2 or 1.5 kN on an area of 500 mm x 500 mm with a minimum factor of safety of 2:1(equivalent to 63 P.S.F.).6).Posts shall be accurately spaced,erected on suitable bases,and maintained plumb.7).324、Runners shall be erected along the length of the scaffold,located on both the inside and the outside posts at even height.Runners shall be interlocked to the inside and the outside posts at even heights.Runners shall be interlocked to form continuous lengths and coupled to each post.The bottom runne325、rs shall be located as close to the base as possible.Runners shall be placed not more than 2.0 m on centers.8).Bearers shall be installed transversely between posts and shall be securely coupled to the posts bearing on the runner coupler.When coupled directly to the runners,the coupler must be kept 326、as close to the posts as possible.9).Bearers shall be at least 10 cm but not more than 30 cm longer than the post spacing or runner spacing.10).Cross bracing shall be installed across the width of the scaffold at least every third set of posts horizontally and every fourth runner vertically.Such Bra327、cing shall extend diagonally from the inner or outer posts or runners upward to the next outer or inner posts or runners.11).Longitudinal diagonal bracing on the inner and outer rows of poles shall be installed at approximately a 45 angle from near the base of the first outer post upward to the extr328、eme top of the scaffold.Where the longitudinal length of the scaffold permits,such bracing shall be duplicated beginning at every fifth post.In a similar manner,longitudinal diagonal bracing shall also be installed from the last post extending back and upward toward the first post.Where conditions p329、reclude the attachment of this bracing to the posts,it may be attached to the runners as close to the posts as possible.12).The entire scaffold shall be tied to and securely braced against the building at intervals not to exceed 9.2 m horizontally and 8.0m vertically.13).Guardrails,mid-rails,toe-boa330、rds,shall be installed on all open sides,elevation and ends of scaffolds.Debris netting will completely enclose all elevations of scaffolds”.All scaffolds will fulfill the requirement of Chinese Law and standards as a minimum.14).Couplers shall be of a structural metal,such as drop-forged steel,mall331、eable iron,or structural grade aluminum.The use of gray cast iron is prohibited.c)Tubular welded frame scaffolds 1).Metal tubular frame scaffolds,including accessories such as braces,brackets,trusses,screw legs,ladders,etc.shall be designed,constructed,and erected to safely support four times the ma332、ximum rated load.2).Spacing of panels or frames shall be consistent with the loads imposed.3).Scaffolds shall be properly braced by cross bracing or diagonal braces,or both,for securing vertical members together laterally,and the cross braces shall be of such length as will automatically square and 333、align vertical members so that the erected scaffold is always plumb,square,and rigid.All brace connections shall be made secure.4).Scaffold legs shall be set on adjustable bases or plain bases placed on mudsills or other foundations adequate to support the maximum rated load.5).The frames shall be placed one on top of the other with coupling or stacking pins to provide proper vertical alignment of
