28、红岩煤矿）位于华蓥市溪口镇，始建于1972年，2001年由地方国营企业改制成私营企业。矿区范围由18个拐点圈闭，矿区面积2.1695km2，允许开采K1、K4煤层，开采深度+915+200m（其中K4煤层开采深度+915+540m）。该矿原生产能力9万吨/年，2004年11月1日，四川省经委以批准立项进行扩建，扩建能力15万吨/年，2006年11月3日正式启动建设，2014年9月5日，项目建设基本完成，经广安市经信委批准进入联合试运转。矿井为平硐开拓、中央分列抽出式通风、大巷机车牵引运输、上山采用绞车提升、10KV双电源入井、自流排水，矿井采用柔性掩护支架采煤法、放炮落煤，掘进采用钻爆法施工、

29、金属架棚支护。矿井属煤与瓦斯突出矿井，矿井主要致灾因素为瓦斯、水患、顶板、运输，建有完善的监测监控、压风、供水、通讯、人员定位和紧急避险等六大系统。企业具备营业执照、采矿许可证。配置由矿长、技术负责人和副矿长等组成的安全管理机构。工程概况：1、项目名称：华蓥市林丰煤炭有限责任公司红岩煤矿技改扩能工程；2、建设单位：华蓥市林丰煤炭有限责任公司；3、设计单位：中煤科工集团重庆设计研究院有限公司；4、施工单位：重庆市巨能集团建筑安装工程有任公司；5、监理单位：四川久恒工程监理咨询有限责任公司；6、概算总投资：7595.76万元；7、设计生产能力：150kt/a，服务年限：17.4a。二、项目审批情况

30、（一）前置条件1、矿权： 本企业前身是岳池县红岩煤，2001年9月私有化改制。2004年4月提交划定矿区范围申请审批，获得采矿许可证，该矿权许可开采煤层只有K1煤层，未包含K4煤层。由于2005年重庆设计研究院提交的扩建工程初步设计和安全专篇要求开采K4煤层作保护层，为此，必须增加K4煤层进入采矿权，2009年9月，提交了新的资源/储量核实报告，获批新版采矿许可证，明确许可开采K1煤层和K4煤层。2、立项：2004年11月1日，原四川省经委批复同意红岩煤矿进行技改扩能立项（川经煤炭函2004586号四川省经委关于同意溪口煤矿、红岩煤矿、李家沟煤矿技改扩能立项的批复）。3、安全预评价：2005年

31、7月8日，四川志强矿山安全科技工程有限责任公司提交了华蓥市林丰煤炭有限责任公司红岩煤矿150kt/a技改工程安全预评价报告，并于当年7月15日，由四川省安全生产评估中心评审通过。4、环境影响评价：2005年7月，重庆渝佳环境影响评价有限公司提交了红岩煤矿技改扩能工程环境影响评价报告书，同年9月，广安市环保局以关于华蓥市林丰煤炭有限责任公司红岩煤矿技改扩能工程环境影响评价报告书的批复（广市环2005267号）批复同意通过该环境影响评价报告书。5、水土保持方案：2008年十一月，华蓥市水电勘测设计室提交了红岩煤矿技改扩能工程水土保持报告书。同年12月，华蓥市水利局以关于红岩煤矿技改扩能工程水土保持

32、的批复（华水200878号）批复同意该方案。（二）初步设计和安全专篇编制和审批1、初步设计和安全专篇编制：2005年9月中煤科工集团重庆设计研究院编制完成了华蓥市林丰煤炭有限责任公司红岩煤矿技改扩能工程初步设计和安全专篇。2、广安市经委于2005年11月9日批准了该项目初步设计（广经200586号关于同意华蓥市红岩煤矿技改扩能工程开工的批复），四川煤监局于2006年10月11日批准了安全专篇（川煤监函2006111号关于四川省华蓥市林丰煤炭有限责任公司红岩煤矿技改扩能初步设计安全专篇的批复）。3、在项目施工过程中，根据揭露煤层位置变化等情况，重庆设计研究院于2012年进行了初步设计和安全专篇修

33、改，四川省经信委于2012年3月26日批准同意初步设计修改（川经信煤炭函2012297号关于同意华蓥市林丰煤炭有限责任公司红岩煤矿技改扩能工程修改初步设计的函），四川煤监局于2012年12月31日批准同意安全专篇修改（川煤监函2012218号关于华蓥市林丰煤炭有限责任公司红岩煤矿技改扩能工程初步设计安全专篇修改说明的批复）。4、该项目在后续实施过程中，由于+719m北底板瓦斯巷施工通风距离过长等情况变化，重庆设计研究院于2013年12月再次对该项目初步设计及安全专篇进行了局部变更，四川煤监局于2014年1月21日批复同意局部变更（川煤监审批201432号）。广安市经信委于2014年6月批复同意

34、变更（关于华蓥市林丰煤炭有限责任公司红岩煤矿扩建工程调整初步设计局部变更说明书的批复）。5、2014年12月20日，重庆设计研究院以设计变更通知书对现场局部一些小的变化进行了设计变更予以确认。（三）工期审批1、首次确定工期：2005年11月9日，原广安市经委下达的开工批复（广经200586号关于同意华蓥市林丰煤炭有限责任公司（红岩煤矿）启动技改扩能工程建设的批复），确定本项目建设工期为42.9个月。2、正式开工时间：2006年11月3日，华蓥市煤管局下达开工通知（华煤管2006109号关于同意华蓥市林丰煤炭有限责任公司（红岩煤矿）启动技改扩能工程建设的批复），本项目正式开工建设。3、工期延续：

35、因工业场地征地及业主资金筹集困难等原因，2010年2月2日，广安市经委同意延长工期至2012年2月28日（广经201032号关于华蓥市林丰煤炭有限责任公司扩建工程延期的批复）。4、因2012年2月修改初步设计新增工程导致增加工期，2012年3月26日，四川省经信委同意增加工期13.6个月，工期顺延至2013年4月（川经信煤炭函2012297号关于同意华蓥市林丰煤炭有限责任公司红岩煤矿技改扩能工程修改初步设计的函）。5、由于2012年8月发生肖家湾煤矿“8.29”事故致政策性强令停工，2013年2月20日，广安市经信委同意该矿建设项目建设工期顺延至2013年8月底（广经201316号关于华蓥市林

36、丰煤炭有限责任公司等4户在建矿井因政策性停工不计建设工期的批复）。6、2013年4月30日，发生华蓥市天池煤矿“4.30”事故，5月11日，发生泸县桃子沟煤矿“5.11”事故，遵照省市政府关于煤矿停止井下一切作业的指令，红岩煤矿建设项目于5月1日起全面停止井下一切作业，2014年3月14日，华蓥市委、市政府领导审批同意红岩煤矿技改项目恢复建设作业（华煤复201422号关于同意华蓥市林丰煤炭有限责任公司恢复建设的通知），2014年4月14日，广安市经信委批复同意华蓥市林丰煤炭有限责任公司扩建工程建设工期顺延至2014年7月14日（广经信发201449号关于华蓥市盛昌煤业有限公司等2户煤矿因政策性

37、停产不计工期的批复）。7、联合试运转工期延续：本项目于2014年9月5日经批准进行联合试运转，9月13日晨我市发生罕见特大暴雨，根据政府指令，停止井下一切作业直至11月6日经华蓥市政府批准方恢复联合试运转，广安市经信委于2014年12月23日以广经信煤炭201444号关于华蓥市林丰煤炭有限责任公司扩建工程因政策性停工不计联合试运转期限的批复同意将联合试运转期限顺延至2015年4月28日。三、项目验收情况（一）联合试运转前的专项验收1、瓦斯抽采系统设计审批和抽采工程竣工验收：2007年9月13日，四川煤矿安全监察局以关于林丰煤炭有限责任公司红岩煤矿矿井瓦斯抽采利用设计的批复同意按四川矿山安全技术

38、培训中心编制的瓦斯抽采利用设计建设本矿瓦斯抽采系统。2010年11月5日，四川煤矿安全监察局以关于华蓥市林丰煤炭有限责任公司瓦斯抽采系统工程竣工验收的批复（川煤监审批2010648号）批复“华蓥市林丰煤炭有限责任公司红岩煤矿技改项目瓦斯抽采系统”通过验收，同意投入使用。2、新水平防治煤与瓦斯突出专项验收：2014年6月12日，华蓥市煤管局组织专家对红岩煤矿新水平防治煤与瓦斯突出进行专项验收，6月25日，以关于对华蓥市林丰煤炭有限责任公司红岩煤矿防治煤与瓦斯突专项验收的批复（华煤管201464号）同意验收通过。（二）联合试运转验收审批2014年7月89日，广安市经信委会同广安市安监局、华蓥市煤管

39、局组织专家组对该建设项目进行了联合试运转验收，建设单位对验收提出的问题组织了整改，2014年9月5日，广安市经信委以关于华蓥市林丰煤炭有限责任公司红岩煤矿扩建工程进行联合试运转的批复（广经信煤炭201430号）批复同意进行联合试运转，华蓥市煤管局于当日以关于同意华蓥市林丰煤炭有限责任公司红岩煤矿扩建工程进行联合试运转的通知（华煤复201432号）同意井下进行联合试运转作业。四、 建设项目单项验收审批进展情况（一）工程施工：本项目土建、矿建和安装工程均由重庆巨能集团建筑工程公司承担。（二）工程质量监理：由四川久恒工程监理咨询有限责任公司承担。（三）工程质量认证：由煤炭工业四川建设工程质量监督站承

40、担，于2015年2月完成认证并提交了华蓥市林丰煤炭有限责任公司红岩煤矿扩建工程工程质量认证报告。（四）主要设备检测：由四川安全生产检测检验技术研究院承担，已检测并于2015年2月提交了检测报告。（五）新水平煤层瓦斯参数测定：由中国矿业大学承担，已完成，提交了林丰煤炭有限责任公司煤层瓦斯参数测定研究报告。（六）矿井水患现状调查：由四川华标勘察设计有限公司承担，已完成，提交了华蓥市林丰煤炭有限责任公司红岩煤矿矿井水患现状调查报告，并在四川省矿山防治水中心备案。（七）通风阻力测定和通风机性能测试：委托四川蜀龙安全评价咨询有限公司承担，已完成并提交了红岩煤矿通风阻力测定报告和红岩煤矿通风机性能测试报告

41、。（八）2014年度瓦斯等级鉴定：已由广安市矿山救护大队组织专家鉴定并提交了华蓥市林丰煤炭有限责任公司红岩煤矿鉴定报告。（九）安全质量标准化建设工作：2014年度申请三级达标验收，已由第三方资质单位组织验收并报广安市安监局认定，于2015年3月公示，现已通过。（十）矿井瓦斯治理工作体系建设和安全高效矿井建设：已由华蓥市煤管局初验收上报广安市安监局。（十一）矿井环保设施竣工验收：验收单位正在编写环保设施现场核查报告，计划4月底验收完。（十二）矿井水土保持验收工作：已完成。2015年3月22日，华蓥市水务局以华水函201513号关于红岩煤矿技改扩能工程水土保持设施竣工验收意见的函同意本项目水土保持

42、设施通过竣工验收。（十三）安全设施预评价：委托四川蜀龙安全评价咨询有限公司承担，正在进行，计划4月20日完成。（十四）安全设施设备竣工验收：计划2015年4月下旬组织安全设施设备竣工验收。（十五）矿井生产能力核定：计划2015年5月上旬申请进行能力核定并颁发安全生产许可证。五、自查自纠总结红岩煤矿技改扩能工程自2006年11月获准同意开工以来，遵守建设秩序，严格按设计组织施工，前期因征地等原因导致工期延误，后由于现场条件发生变化，进行了设计变更。后期由于2012年发生“8.29”肖家弯特大事故和2013年发生“5.11”桃子沟特大事故导致四川省煤炭行业全面整顿，使建设工期延长至2014年7月4日，我矿仍坚持如期完成建设内容，并提交联合试运转验收，于2014年9月5日获准进行联合试运转，由于遇到“9.13”特大暴雨，根据政策停工，在复工整改期间，我矿严格按要求进行隐患整改，按程序申报验收，于11月4日批准恢复联合试运转。联合试运转期间，我们严格按联合试运转方案进行，严格按批准的“两面四头”组织试运转作业，侧重于职工队伍建设和系统整改调试，查漏补缺，并抓紧时间完成项目单项验收认证等工作的推进。到目前为止，本建设项目在建设期间和试运转期间无违反程序行为称违规开采行为，在主管部门的领导和帮助下取得了建设项目阶段性的成果。根据目前进度和计划，力争于5月完成其余验收工作。7
